Work Environment

The work environment is dynamic and collaborative, filled with opportunities for meaningful interactions with patients. As part of our onsite healthcare team, you will work closely with fellow clinicians and community partners. The typical work hours are from 9:00 a.m. to 6:00 p.m. and may occasionally extend to evening clinics until 8:00 p.m. Additionally, you may participate in community outreach programs or health fairs over weekends, contributing directly to community health initiatives.

Key Responsibilities

• Conduct comprehensive health assessments and screenings for individuals and families during home visits, ensuring all health needs are thoroughly evaluated.
• Diagnose and manage both acute and chronic medical conditions, focusing on preventive care and health promotion tailored to individual patient needs.
• Develop and implement culturally sensitive health education programs in collaboration with community organizations, aiming to boost health literacy among diverse populations.
• Advocate for patients' rights and access to necessary healthcare services, ensuring that all individuals receive appropriate and timely care.
• Coordinate referrals and follow-up care for patients, maintaining a seamless continuum of care to enhance patient outcomes.

Qualifications

• Current Nurse Practitioner certification and an active state nursing license.
• A Bachelor’s or Master’s degree in Nursing from an accredited institution.

Compensation and Benefits
