Key Responsibilities

• Conduct thorough health assessments and screenings for individuals and families in the community to evaluate their health needs and determine appropriate care plans.
• Diagnose and manage a range of acute and chronic medical conditions, concentrating on preventive strategies and health education to promote overall wellness.
• Collaborate with local community organizations to develop and implement culturally sensitive health education programs tailored to our diverse populations.
• Advocate for patient rights, ensuring that all individuals have equitable access to necessary healthcare services and support.

Patient Population / Client Focus

Qualifications

• Possession of a valid and current certification as a Nurse Practitioner.
• An active nursing license in the state of New York.
• A Master’s degree in Nursing from an accredited program.
• A minimum of three years of clinical experience, ideally within community health or public health environments.
• Exceptional interpersonal and communication skills, with an ability to connect with patients from diverse backgrounds.
• A strong commitment to social justice and health equity, particularly in advocating for underrepresented communities.

Preferred Experience

• Experience working in assisted living or similar healthcare settings.

Compensation and Benefits

• A competitive salary range of USD 180,000 – 200,000 per year, commensurate with your experience and qualifications.
