Work Environment

This position is onsite at a school facility in Uniondale, New York, where you will work closely with educational staff and administrators to ensure the well-being of students. Key Responsibilities

• Conduct comprehensive initial patient histories and perform detailed physical examinations to assess students' health needs.
• Formulate, document, and communicate individualized care plans for patients, ensuring that all necessary screenings and preparations are executed effectively.
• Carry out daily patient rounds, proactively identifying health concerns and monitoring the well-being of ongoing cases.
• Prescribe medications, order laboratory tests, and request additional diagnostic procedures as needed based on patient assessments.
• Analyze laboratory results and diagnostic test findings to inform treatment decisions and adjust care plans accordingly.
• Collaborate with attending physicians, nursing staff, and other healthcare professionals to uphold the highest standards of patient care.
• Prepare comprehensive discharge summaries and ensure that patients understand their post-discharge instructions to facilitate a smooth transition of care.
• Participate actively in continuous quality improvement initiatives, research projects, and educational activities to enhance professional growth and team performance.

Patient Population / Client Focus

This position primarily focuses on the school-aged population, addressing their unique healthcare needs in a supportive educational setting. You will be involved in managing acute and chronic conditions, conducting health screenings, and providing health education to students, parents, and school staff. Your role is pivotal in fostering a positive environment where students feel safe and supported in their health journey.

Qualifications

• A Master’s Degree in a Nurse Practitioner Program is required, along with National Board Certification in your specialty area.
• Must possess a current and valid license to practice as a Nurse Practitioner and Registered Professional Nurse in New York State.
• A collaborative practice agreement with designated protocols filed with the New York State Department of Education, or collaborative relationship attestation compliant with the Nurse Practitioner Modernization Act 2015.
• Current Basic Life Support (BLS) certification is mandatory.

Preferred Experience

• Previous experience in a school nursing or pediatric setting is advantageous.
• Familiarity with electronic health records (EHR) systems to ensure seamless patient care management.
• Experience with performance improvement initiatives focused on enhancing patient care processes and outcomes.

Compensation and Benefits

The Nurse Practitioner role offers an attractive salary range of USD 150,000 to 160,000 per year, commensurate with experience and qualifications.
