Key Responsibilities

• Conduct thorough initial patient histories and perform detailed physical examinations, ensuring a comprehensive understanding of each patient's health status.
• Develop, document, and communicate individualized care plans, coordinating closely with patients and their families throughout the treatment process.
• Perform daily patient rounds to monitor progress, adjust treatment plans as necessary, and respond to patient needs promptly.
• Order and interpret laboratory tests, diagnostic imaging, and other assessments to facilitate accurate diagnosis and effective treatment strategies.
• Collaborate with attending physicians, residents, nursing staff, and allied healthcare providers to ensure a multidisciplinary approach to patient care.
• Prepare thorough discharge summaries, write prescriptions, and make appropriate referrals, while ensuring patients receive clear post-discharge care instructions.
• Engage in continuous professional development activities, including participation in performance improvement initiatives, research, and educational programs.
• Perform related duties as required, contributing to a holistic approach to patient care, as outlined in the essential functions of the role under the Americans with Disabilities Act.

Patient Population / Client Focus

The Nurse Practitioner will primarily care for adult patients with a variety of medical conditions common in internal medicine. You will have the opportunity to work with patients experiencing chronic illnesses, acute medical issues, and preventive healthcare needs, collaborating closely with them to manage their health effectively. Qualifications

• Master’s Degree in a Nurse Practitioner program and board certification in internal medicine or related specialty area are required.
• Active and current Nurse Practitioner license to practice in New York State, as well as a valid Registered Professional Nurse license in New York State.
• Must possess a collaborative practice agreement with established protocols filed with the New York State Department of Education, or provide evidence of a collaborative relationship attestation per the Nurse Practitioner Modernization Act of 2015.
• Current Basic Life Support (BLS) certification is required.

Preferred Experience

While not mandatory, candidates with prior experience in a hospital or outpatient internal medicine setting will be given preference. Experience with electronic medical records (EMR) systems, familiarity with coding procedures, and proficiency in performing specific clinical procedures will enhance your candidacy.

Compensation and Benefits

This position offers an annual salary ranging from USD 150,000 to 160,000, commensurate with experience and qualifications.
