Key Responsibilities

• Conduct thorough patient assessments through interviews, physical examinations, and analysis of medical histories, ensuring a complete understanding of their health needs.
• Collaborate closely with physicians to develop, implement, and monitor individualized patient management plans that address both short-term and long-range health objectives.
• Provide continuity of care through regular evaluations and adjustments of patient management plans, ensuring our residents receive the highest standard of care.
• Educate patients and families on managing internal medicine conditions, fostering an understanding of treatment plans, symptoms to monitor, and when to seek further medical assistance.
• Document all patient interactions and clinical findings accurately in our electronic health record (EHR) system, ensuring compliance with organizational standards and facilitating seamless communication among the care team.
• Participate in regular team meetings and case discussions, contributing invaluable insights to improve patient care and operational efficiency.

Patient Population / Client Focus

The primary focus of this position is on elderly patients residing in our Assisted Living Facility. These individuals often have complex health needs, including chronic conditions typical of aging populations, such as diabetes, hypertension, and cardiovascular diseases. As a key member of our healthcare team, you will provide tailored care aimed at enhancing their quality of life while promoting independence and dignity.

Qualifications

• Graduate of an accredited Nurse Practitioner or Physician Assistant program.
• A valid Nurse Practitioner or Physician Assistant license in the state of New York.
• Completion of the Physician Assistant National Certifying Examination (PANCE) if applicable.
• Strong interpersonal communication skills and a proactive approach to patient care.
• Ability to work collaboratively within a multidisciplinary team.

Preferred Experience

• A minimum of 2 years of experience in an ambulatory or assisted living care setting, preferably with a focus on internal medicine.

Compensation and Benefits
