Key Responsibilities

• Conduct thorough patient assessments, including detailed interviews and comprehensive physical examinations.
• Review and update medical histories, enabling the formulation of precise diagnoses and effective treatment plans.
• Collaborate with physicians in developing both short-term and long-term management strategies for patient care.
• Implement individualized patient management plans to ensure continuity of care.
• Monitor and evaluate treatment progress, making necessary adjustments based on patient response and evolving health status.
• Act as an advocate for residents, ensuring their needs and preferences are prioritized in care planning.
• Provide patient and family education on managing health conditions, medications, and healthy lifestyle choices.
• Maintain accurate and timely documentation of patient interactions, treatment plans, and outcomes in compliance with healthcare regulations.
• Participate in regular team meetings to discuss patient care strategies and quality improvement initiatives.

Patient Population / Client Focus

This role primarily involves caring for elderly patients residing in our Assisted Living Facility. Our residents often have complex medical needs, including chronic illnesses and mobility impairments. As an Internal Medicine NP/PA, you will be at the forefront of managing their health, ensuring they receive coordinated and holistic care that addresses their physical, emotional, and social well-being. Your expertise will enable you to significantly improve the quality of life for the elderly population we serve.

Qualifications

• Graduation from an accredited Nurse Practitioner or Physician Assistant program.
• Current license to practice as a Nurse Practitioner or Physician Assistant in New York State.
• A minimum of two years of experience in an ambulatory care setting is preferred.
• Completion of the Physician Assistant National Certifying Examination (if applicable).
• Strong interpersonal skills and the ability to communicate effectively with patients, families, and team members.
• Proficient in using Electronic Health Records (EHR) systems for reporting and documentation.

Preferred Experience

• Experience working in geriatrics or internal medicine is highly desirable.
• Demonstrated ability to work effectively in a team-oriented environment.
• Previous experience in a long-term care or assisted living environment is a plus.
• Certification as a Nurse Practitioner is preferred.

Compensation and Benefits
