Work Environment

This role is onsite, allowing you to work directly with patients in an outpatient setting. You will be part of a cohesive team that includes physicians, other nurse practitioners, physician assistants, medical assistants, dedicated medical scribes, and care managers. Key Responsibilities

As a Nurse Practitioner, you will have multifaceted responsibilities that include:

• Providing exceptional primary care services to patients, addressing routine and acute health issues during office visits.
• Administering annual wellness visits and conducting comprehensive health risk assessments, ensuring thoughtful and specific documentation.
• Maintaining seamless care coordination with specialists, testing facilities, and community health agencies.
• Leading population health initiatives, such as ensuring eligible patients receive timely evidence-based screenings.
• Supporting the care team with phone triage and proactive outreach, addressing patient concerns promptly.
• Educating patients about their chronic health conditions, care plans, and available treatments to empower their health journey.
• Participating in community outreach and promotional activities to enhance the visibility of our services.
• Conducting home visits for patients who are unable to travel to the office, ensuring accessibility to care.
• Reporting directly to the Medical Director while collaborating closely with operational leadership for process improvement.

Patient Population / Client Focus

Your primary focus will be on older adults, particularly those enrolled in Medicare, as we strive to provide personalized care that supports their health and well-being. Qualifications

To qualify for this role, candidates must possess the following:

• A Master of Science in Nursing, Doctor of Nursing Practice, or a graduate from an NP Certificate Program.
• National certification as a Family, Adult, or Adult-Gerontology Primary Care Nurse Practitioner through AANP or ANCC.
• An active, non-probationary state Nurse Practitioner license.
• An active DEA license.
• Experience in primary care settings, particularly in internal medicine, geriatrics, or family medicine.
• Knowledge of HCC documentation, ICD-10 coding, and health risk assessments.
• A strong desire to collaborate and work as part of a multi-disciplinary care team.
• Bilingual proficiency is highly desirable to effectively serve our diverse patient population.

Preferred Experience

While not mandatory, candidates with the following backgrounds will stand out:

Compensation and Benefits

The salary for this position ranges from USD 115,000 to USD 150,000 annually, commensurate with experience and specialized expertise. Additional benefits include:
