Key Responsibilities

• Deliver exceptional primary care services by conducting comprehensive office visits for both routine and acute medical issues.
• Administer annual wellness visits and health risk assessments, requiring the ability to synthesize patient data and document findings accurately for continuity of care.
• Coordinate care with specialists and testing facilities, ensuring an integrated approach to patient health.
• Lead initiatives in population health, such as ensuring all eligible females receive evidence-based breast cancer screenings every two years.
• Assist with phone triage and outreach efforts to improve patient engagement and accessibility.
• Provide education to patients on their health conditions, care plans, and available treatments, empowering them to manage their health effectively.
• Engage in home visits as necessary to evaluate patient conditions and enhance care delivery.
• Collaborate with the medical director and operational leadership to continuously improve practice standards and enhance patient experiences.

• An active, non-probationary Nurse Practitioner license in the state of New York.

Compensation and Benefits
