Work Environment

The position is primarily onsite at a local school in Huntington Station, promoting a collaborative atmosphere with a focus on primary care. As part of a multifaceted healthcare team, you’ll work closely with physicians, nurses, counselors, and administrative staff, all dedicated to improving health outcomes in an educational context. Key Responsibilities

• Conduct thorough patient assessments by interviewing students, performing physical examinations, and reviewing medical histories.
• Collaborate with physicians to develop and implement individualized patient management plans that address both immediate and long-term health needs.
• Educate patients and their families on disease prevention and management, ensuring they are informed about their health conditions.
• Monitor and document patient progress in compliance with school health policies and procedures, utilizing electronic health record systems effectively.
• Participate in health screenings and public health promotions within the school, contributing to an awareness of physical and mental health issues.
• Provide compassionate continuity of care, addressing any emerging health concerns promptly.

Patient Population / Client Focus

This role serves a diverse school community, including children and adolescents, and is crucial in addressing both common and complex health issues that arise in educational settings. Qualifications

• A Bachelor’s degree in Nursing or a related field from an accredited institution.
• Current Nurse Practitioner or Physician Assistant license in the state of New York.
• Certification from the appropriate national board (e.g., National Commission on Certification of Physician Assistants).

Preferred Experience

• A minimum of two years of experience in an ambulatory care setting, preferably within a school or community health context.
• Experience using electronic health record (EHR) systems and familiarity with telemedicine capabilities is a plus.
• Demonstrated ability in developing health promotion programs tailored for youth.

Compensation and Benefits
