Work Environment

The ideal candidate will work onsite at our state-of-the-art hospital facility in Babylon, which is located in a vibrant community that values accessible healthcare. As part of a supportive team consisting of physicians, physician assistants, medical assistants, registered nurses, and care managers, you will play a pivotal role in enhancing the patient experience while delivering exceptional primary care.

Key Responsibilities

• Conduct comprehensive office visits for both routine check-ups and acute health issues.
• Administer annual wellness visits and health risk assessments, focusing on holistic patient evaluations and precise documentation.
• Coordinate care with various providers, specialists, and testing facilities, ensuring seamless transitions for patients requiring specialized services.
• Lead population health initiatives, such as making certain that eligible patients receive vital screenings and preventive care.
• Provide phone triage and outreach support, assisting the care team in managing patient needs effectively.
• Educate patients thoroughly on their health conditions, care plans, and available treatments to empower them in their healthcare journey.
• Engage in community outreach and promotional activities to enhance health awareness and outreach.
• Perform home visits when necessary, particularly for patients requiring additional support.
• Report directly to the Medical Director, working closely with operational leadership to provide input on improving clinical outcomes and patient satisfaction.

Patient Population / Client Focus

The primary focus of this role is to provide individualized care to older adults on Medicare who often deal with complex health conditions. By implementing thoughtful, evidence-based strategies, you will assist patients in managing chronic illnesses, navigating healthcare needs, and ultimately ensuring they lead healthier, more fulfilling lives.

Qualifications

• A Master of Science in Nursing, Doctor of Nursing Practice, or graduate of an accredited Nurse Practitioner Certificate Program.
• National certification as a Family, Adult, or Adult-Gerontology Primary Care Nurse Practitioner through the AANP or ANCC.
• Possession of an active, non-probationary state Nurse Practitioner license.
• Active DEA license for controlled substance prescription.
• Demonstrated experience in primary care settings, particularly strong in internal medicine, geriatrics, or family medicine.
• Familiarity with coding systems such as HCC (Hierarchical Condition Categories) and ICD-10.

Preferred Experience

• Experience with health risk assessments (HRAs) and the implementation of preventive health strategies in patient care.
• A proven ability to collaborate with diverse teams across healthcare disciplines.
• Strong communication skills, with bilingual proficiency being a significant asset.
