The practice operates chiefly in a concierge medicine model, allowing for a more personalized interaction with patients through home visits as needed.

Key Responsibilities

• Conduct comprehensive initial patient histories and thorough physical examinations to accurately assess health concerns.
• Develop, document, and communicate individualized care plans, following up diligently to ensure patients meet their health goals.
• Perform daily patient rounds, focusing on continuity of care and timely responding to any medical issues that arise.
• Utilize electronic health record systems to enter orders for medications, laboratory testing, and diagnostic procedures efficiently.
• Interpret laboratory results and collaborate with other healthcare professionals to optimize patient outcomes through integrated care.
• Prepare discharge summaries which encompass treatment notes, prescriptions, and referrals, ensuring patients have clear post-care instructions.
• Actively participate in patient education regarding health maintenance and disease prevention, addressing concerns and facilitating understanding.
• Perform other related duties as required, adhering to the highest standards set by the Americans with Disabilities Act in all job responsibilities.

Qualifications
