Position Overview

The Gundersen Health and Wellness Clinic is actively seeking a skilled Nurse Practitioner (NP) to join our dedicated team within the Center for Advanced Cardiac Care (CACC). This full-time role will allow you to play a pivotal part in transforming patient care within our outpatient practice. Under the guidance of the Chief Nurse Practitioner and the direction of the CACC Medical Director, you will work in a collaborative environment designed to support advanced cardiovascular health.

Work Environment

This position is onsite at our Assisted Living Facility, allowing for an engaging and dynamic work setting where you will interact with patients and their families regularly. As part of a team that includes Physicians, fellow Nurse Practitioners, Registered Nurses, and a wealth of supportive administrative staff, you will help create an environment that emphasizes holistic care and wellness for our patients.

Key Responsibilities

• Conduct detailed patient evaluations, including obtaining histories and performing both comprehensive and targeted physical examinations.
• Order and interpret diagnostic tests; manage both chronic and acute health issues across a diverse patient population.
• Utilize established protocols to devise individualized, goal-oriented care plans that incorporate health maintenance and patient education, ensuring they include essential medical and nursing interventions.
• Work collaboratively in developing treatment plans with physicians, regularly reassessing and adapting plans based on patient responses.
• Provide guidance and education to patients and their families regarding disease processes and self-management strategies, focusing on enhancing their understanding and involvement in their own health.
• Facilitate patient admissions to the hospital and coordinate with inpatient teams to guarantee safe patient discharges.
• Participate actively in daily team meetings, collaborating with various specialists, including Nutritionists, Social Workers, and Pharmacists, to provide integrated care.
• Document all patient interactions rigorously within the electronic medical record (EMR) system, ensuring accurate and timely communication within the care team.
• Engage in research initiatives, disseminating findings and integrating evidence-based practices into daily operations to elevate patient care standards.
• Maintain a therapeutic and consumer-oriented environment, ensuring that all interactions are safe and conducive to healing.

Patient Population / Client Focus

You will focus on a diverse patient demographic, including individuals with advanced heart failure, pulmonary arterial hypertension, and those receiving or being evaluated for post-heart transplant care. By implementing tailored interventions, you will help improve outcomes and support patients in navigating their health journeys.

Qualifications

• A Master's degree in Nursing with Nurse Practitioner accreditation, specifically focused on adult health.
• A minimum of two years of experience as an NP in a hospital or outpatient setting, ideally with a focus on cardiology.
• An active New York State Registered Professional Nurse license and Nurse Practitioner license.
• Board certification as a Nurse Practitioner and current certifications in BCLS, ACLS, DEA, and NPI.
• Proficiency in electronic medical record systems and standard office software—including MS Word, MS Excel, and MS PowerPoint—for effective documentation and communication.

Preferred Experience

Experience managing complex patient populations, including those receiving advanced therapies such as outpatient intravenous treatments or mechanical circulatory support systems, is highly desirable. Familiarity with clinical protocols related to cardiology will enhance your ability to contribute meaningfully to our team and patient care.

Compensation and Benefits

The compensation for this role ranges from USD 120,000 to 220,000 annually, commensurate with experience and qualifications.
