Key Responsibilities

• Conduct comprehensive assessments by taking detailed patient histories and performing thorough physical examinations.
• Develop, document, and implement individualized care plans while actively communicating with patients about their progress.
• Perform daily patient rounds, ensuring ongoing assessment and adjustment of care plans as needed.
• Order and interpret laboratory tests, diagnostic imaging, and other necessary evaluations to guide clinical decision-making.
• Collaborate with multidisciplinary teams, including physicians, nursing staff, and specialists to optimize patient care and outcomes.
• Document discharge summaries upon patient discharge, including prescriptions, follow-up appointments, and additional patient instructions.
• Educate patients on post-discharge care, lifestyle modifications, and follow-up strategies to support health management.
• Participate in the continuous improvement of care protocols and processes to enhance patient experience and satisfaction.

Qualifications

• Master’s Degree from an accredited Nurse Practitioner program.
• Current licensure to practice as a Nurse Practitioner and Registered Professional Nurse in New York State.
