Key Responsibilities

• Provide health education and counseling to patients, emphasizing preventive care and chronic disease management.
• Develop, implement, and communicate individualized treatment plans based on patient assessments, ensuring alignment with their health goals.
• Order, interpret, and act upon lab and diagnostic test results to adjust treatment plans and provide evidence-based care.
• Maintain thorough and compliant documentation in the electronic medical record system, ensuring accuracy and adherence to regulatory standards.
• Collaborate with clinical leadership and operational teams to set and achieve strategic goals related to patient care and organizational performance.
• Participate in initiatives that promote value-based care, focusing on improving health outcomes while managing costs effectively.
• Deliver exceptional customer service, cultivating strong patient relationships and fostering high levels of satisfaction and retention.
• Practice within the ethical standards and guidelines provided by the nursing profession and maintain Board Certification compliance.
• Address prescriptions and recommendations from pharmacists while providing comprehensive medications management for residents.
• Coordinate care efforts with primary care providers, specialists, and telemedicine resources to ensure comprehensive patient management.
• Conduct acutely needed visits and participate in family conferences, offering advanced care planning and support when necessary.
• Assess, diagnose, and manage both chronic and acute health conditions with an individualized approach that supports each resident's needs.

Patient Population / Client Focus

This role primarily caters to seniors living in assisted living and independent communities, as well as skilled nursing facilities. You will engage with a diverse patient population, addressing various health challenges associated with aging and chronic diseases. Qualifications

• Possession of national certification from the American Association of Nurse Practitioners (AANP) or the American Nurses Credentialing Center (ANCC).
• Valid and unrestricted state license to practice as a Nurse Practitioner.
• Active DEA license.
• Eligibility to work in the United States.
• Master of Science in Nursing (MSN), Doctor of Nursing Practice (DNP), or graduation from a Nurse Practitioner certificate program.

Compensation and Benefits
