Key Responsibilities

• Conduct thorough health assessments and comprehensive physical examinations of patients to determine health status and identify any health issues.
• Order, perform, and interpret a wide range of diagnostic tests including laboratory tests and imaging studies to accurately assess conditions.
• Collaborate with other healthcare professionals such as pediatricians, mental health specialists, and school nurses to develop and implement patient-centered care plans.
• Prescribe necessary medications and develop treatment plans tailored to each patient's needs while monitoring effectiveness and making adjustments as necessary.
• Provide health education and counseling to patients and their families to empower them with knowledge about health promotion and disease prevention.
• Engage actively in team meetings to review patient cases and progress, ensuring a collaborative approach in care delivery.
• Maintain meticulous documentation of patient encounters, treatment plans, and care outcomes to ensure continuity of care and compliance with medical standards.
• Participate in quality improvement initiatives that focus on enhancing patient care outcomes and program efficiencies.

Patient Population / Client Focus

Your primary focus will be on promoting the health and well-being of school-aged children and their families. This role requires a keen understanding of the unique healthcare needs of this demographic, including managing chronic conditions, addressing acute health concerns, and supporting mental health initiatives.

Qualifications

• A valid and unrestricted Nurse Practitioner license in the state of New York.
• A minimum of 2 years of experience as a Family Nurse Practitioner, with a strong emphasis on pediatric care.
• In-depth knowledge of best practices in healthcare processes, legal regulations, and ethical guidelines in nursing.
• Outstanding clinical assessment, diagnostic, and decision-making skills.
• Excellent ability to communicate effectively with patients, families, and a multidisciplinary healthcare team.
• Proficient organizational and time management skills to handle a busy patient load effectively.
• Experience with electronic health record (EHR) systems to manage patient data systematically.

Preferred Experience

Compensation and Benefits
