Lifeline Family Care Center is actively seeking a dedicated Family Nurse Practitioner (FNP) to join our team in Babylon, New York. This is a full-time position primarily focused on providing comprehensive health services to our patients in a home care setting. Work Environment

This full-time position is based onsite at our facility in Babylon. The working conditions are designed to support a balanced life for our staff, with no on-call duties or weekend shifts required. Typical hours of operation are Monday, Wednesday, and Friday from 8:00 a.m. to 5:00 p.m. By providing a structured schedule, we ensure our team can maintain their well-being while delivering care.

Key Responsibilities

• Conduct thorough health assessments and evaluations, ensuring comprehensive primary care services tailored to individual patient needs.
• Perform onsite health risk assessments to identify potential health issues, developing and implementing appropriate intervention plans.
• Educate patients on wellness and preventive measures, providing resources for healthy lifestyle choices that enhance overall health.
• Encourage and guide individuals in making positive lifestyle changes, helping them to embrace healthier habits.
• Assist patients in managing chronic conditions through tailored treatment plans and ongoing monitoring to ensure the best disease management outcomes.
• Oversee and coordinate care delivered by the medical staff within the Marathon Health Center, fostering a collaborative team environment.
• Actively promote health outcomes and patient engagement by participating in community wellness events and health fairs.
• Collaborate with other healthcare professionals within the organization to ensure seamless patient care and support.

Patient Population / Client Focus

Our Family Nurse Practitioner will primarily serve members of the community who are part of CEBT's health and wellness program. This diverse patient population includes individuals of various ages, backgrounds, and health statuses, requiring a compassionate approach tailored to each patient's unique needs.

Qualifications

• A Master’s degree in Nursing with a specialization as a Family Nurse Practitioner is required.
• Must possess current and unrestricted state licensure to practice as a Nurse Practitioner.
• At least two years of experience as a Family Nurse Practitioner in a primary care or family practice setting is essential.
• Proficiency in Electronic Medical Records (EMR) systems is preferred, as it is vital for maintaining accurate patient documentation and care tracking.

Preferred Experience

• Experience working within a home care setting or managing patients with chronic conditions is highly desirable.
• Demonstrated ability to effectively communicate and collaborate with a variety of healthcare professionals.
• Willingness to engage with patients and community members through participation in wellness fairs and educational events.

Compensation and Benefits
