Brightwood Orthopedic and Family Medicine Care is seeking a dedicated Full-time Nurse Practitioner specializing in Family Medicine to join our compassionate team in Islip, New York. Work Environment

This position is onsite in our outpatient practice located in Islip, NY. You'll be part of a collaborative care team comprising other nurse practitioners, physician assistants, medical assistants, and support from registered nurses and care managers. Together, you will work in a supportive and dynamic environment dedicated to improving patient health through coordinated efforts. The typical work schedule is full-time, Monday through Friday, from 8:00 AM to 5:00 PM.

Key Responsibilities

• Deliver exceptional primary healthcare by conducting both routine and acute patient visits.
• Administer Annual Wellness Visits and Health Risk Assessments, emphasizing holistic care and meticulous documentation to ensure compliance and quality.
• Engage in care coordination with other healthcare providers, specialists, testing facilities, and agencies to streamline patient care pathways.
• Drive population health initiatives, such as ensuring timely screenings and assessments for patients, guided by evidence-based practices.
• Participate in phone triage and patient outreach to continuously address health concerns and follow up on care plans.
• Educate patients about their health conditions, treatment options, and care plans, facilitating active participation in their health management.
• Conduct home visits as needed, catering to patients with limited mobility or access to transportation.
• Collaborate with the care team to enhance service quality and patient satisfaction.
• Report directly to the Medical Director and closely interact with operational leadership.

Patient Population / Client Focus

Your primary focus will be on adult patients, predominantly those covered by Medicare, ensuring they receive personalized primary care. Our patient-centered approach allows for meaningful interactions, fostering an atmosphere where patients feel valued and understood. The care you provide aims to keep these individuals healthy and engaged in their own well-being, reducing unnecessary hospital visits.

Qualifications

• Master of Science in Nursing, Doctor of Nursing Practice, or Completion of an accredited NP Certificate Program.
• National certification as a Family, Adult, or Adult-Gerontology Primary Care Nurse Practitioner accredited by AANP or ANCC.
• Active, non-probationary Nurse Practitioner license in the state of New York.
• A valid DEA license for prescribing necessary medications.
• Familiarity with HCC documentation, ICD-10 coding, and Health Risk Assessments.
• A collaborative spirit and a passion for working in a team-oriented healthcare environment.

Preferred Experience

• Experience in primary care settings, particularly in geriatrics, internal medicine, or family medicine.
• Bilingual proficiency may be advantageous in assisting diverse patient populations and enhancing communication.

Compensation and Benefits

This position offers a competitive annual salary ranging from USD 115,000 to 150,000, commensurate with your experience and expertise. Additionally, our benefits package includes:
