Position Overview

We are seeking passionate and dedicated Nurse Practitioners specializing in Adult-Gerontology to join our team at Center Point Healthcare Solution Management Practice. In this full-time role, you will play a critical part in delivering exceptional, value-based primary care to our Medicare patients in Babylon, New York.

Work Environment

This position is located onsite at our hospital-based practice in Babylon, New York, where you will work alongside a collaborative care team made up of physicians, nurse practitioners, physician assistants, and support staff. Key Responsibilities

• Deliver exceptional primary care through thorough patient assessments and understand their unique healthcare needs.
• Conduct comprehensive office visits addressing both routine health and acute medical issues, ensuring the utmost care.
• Administer annual wellness visits and identify health risks through detailed health assessments, emphasizing precise documentation.
• Coordinate care with other healthcare providers, specialists, testing facilities, and community agencies to streamline the patient journey.
• Lead initiatives in population health, ensuring practices such as routine breast cancer screenings are adhered to for eligible patients.
• Assist in phone triage and outreach, supporting preventative health measures and effective patient communication.
• Educate patients on their medical conditions, treatment plans, and healthy lifestyle choices to foster proactive health management.
• Engage in community outreach and promotional activities that align with our mission of equitable healthcare.
• Conduct home visits as necessary to ensure consistent care delivery and support for our patients in their living environments.
• Additional duties may be assigned by the Medical Director, where you will also work closely with the operational leadership.

Patient Population / Client Focus

Your primary focus will be on adults and older adults with a strong emphasis on those enrolled in Medicare. Qualifications

• Master of Science in Nursing, Doctor of Nursing Practice, or completion of a Nurse Practitioner Certificate Program.
• National certification as a Family, Adult, or Adult-Gerontology Primary Care Nurse Practitioner through AANP or ANCC is required.
• A current, non-probationary Nurse Practitioner license in New York.
• Possession of an active DEA license is essential for this role.
• Experience in primary care settings such as internal medicine, geriatrics, or family medicine.
• Familiarity with HCC documentation guidelines, ICD-10 Coding, and Health Risk Assessments (HRAs).
• Strong collaborative spirit with a genuine passion for team-based care across disciplines.

Preferred Experience

• A background of working within value-based care models or an eagerness to implement innovative care approaches.
• Experience using Electronic Health Records (EHR) to track patient interactions and care plans will be considered beneficial.
• Prior experience with community outreach and health education will enhance your application.

Compensation and Benefits

This full-time Nurse Practitioner position offers a competitive salary range of USD 120,000 to 160,000 annually, commensurate with experience and qualifications.
