Work Environment

This position is based onsite within a school facility in Lindenhurst, providing a unique opportunity to interact closely with students and their families. You will work within a collaborative team of professionals dedicated to student health and wellness, where a supportive and engaging atmosphere nourishes growth and effectiveness. Your typical work schedule will range from 9:00 a.m. to 6:00 p.m., with occasional evening clinics extending until 8:00 p.m. Participation in community events or health fairs on weekends may also be required, although an average workweek will total approximately 40 hours.

Key Responsibilities

• Conduct comprehensive health assessments and screenings for children and their families in the school environment, identifying physical, emotional, and social health needs.
• Diagnose and manage both acute and chronic medical conditions, providing preventive care and health promotion tailored to the school-aged population.
• Collaborate with local organizations and school staff to develop and implement effective health education programs aimed at improving student well-being.
• Deliver culturally sensitive care that advocates for the rights and access of students to necessary healthcare services.
• Coordinate patient referrals and follow-up care to ensure students receive continuity of care both inside and outside the school setting.
• Maintain accurate medical records and documentation in compliance with healthcare regulations and standards.
• Engage in community outreach events, providing vital health screenings and raising awareness on preventive health topics.
• Stay current with evolving community health trends, evidence-based practices, and educational policies that affect school healthcare.

Patient Population / Client Focus

Your primary focus will be on school-aged children and their families, addressing a wide variety of physical and mental health issues. You will provide care that not only meets clinical needs but also recognizes the broader social determinants of health that affect this population. Working in a school setting allows you to educate families and empower students in making informed health decisions, fostering a culture of wellness and resilience.

Qualifications

• A Master’s or Doctoral degree in Nursing from an accredited institution is required.
• Certification as a Family Nurse Practitioner (FNP), Adult-Gerontology Nurse Practitioner (ANP), or Adult-Gerontology Primary Care Nurse Practitioner (AGNP) from recognized bodies such as the American Nurses Credentialing Center (ANCC) or the American Academy of Nurse Practitioners (AANP).
• A current, unrestricted Nurse Practitioner (APRN) license in New York State.
• Possession of a valid DEA# and NPI#.
• Strong clinical assessment skills, ability to diagnose and treat a variety of health concerns, and proficiency in electronic health records (EHR) systems.
• Excellent communication and interpersonal skills with an ability to build trust and rapport with diverse populations.
• Proven ability to work independently while also functioning effectively as part of a collaborative healthcare team.

Preferred Experience

• A minimum of 3 years of clinical experience as a Nurse Practitioner, preferably within community health or school-based settings.
• Experience in primary care or family medicine, with an emphasis on pediatric care.
• Demonstrated experience in community outreach and health education initiatives.

Compensation and Benefits

The salary range for this position is between $150,000 and $160,000 per year, reflecting the organization's commitment to attracting talented healthcare professionals.
