Key Responsibilities

• Conduct comprehensive assessments of patients during home visits, evaluating their mental health conditions and determining the necessary treatment plans.
• Facilitate hospital admissions, transfers, and discharges, orchestrating the continuum of care across inpatient and outpatient settings.
• Coordinate patient care activities in collaboration with the healthcare team, focusing on education, counseling, and effective discharge planning.
• Perform telephone triage and assessments for acute clinical issues, providing timely and appropriate recommendations and interventions.
• Assist in formulating, implementing, and evaluating care standards and practice protocols, aligning with institutional policies and regulatory requirements.
• Document patient interactions meticulously using electronic health records, ensuring accuracy and compliance with healthcare regulations.

Patient Population / Client Focus

Your patient population will include individuals with various behavioral health needs, ranging from anxiety and depression to more complex psychiatric conditions. By providing in-home consultations, you will have the opportunity to build rapport and trust with your patients in a comfortable and familiar setting. Qualifications

• A Master of Science in Nursing, Doctor of Nursing Practice, or NP Certificate Program graduate.
• Board certification in Family Medicine or Internal Medicine.
• An active and unrestricted nursing license in New York State.
• US work authorization.
• Strong customer service orientation and a willingness to go above and beyond for patient satisfaction.

Compensation and Benefits

The Nurse Practitioner role offers a competitive salary ranging from $150,000 to $160,000 annually, based on your caseload and experience.
