Key Responsibilities

• Conduct initial patient evaluations by obtaining thorough medical histories and performing comprehensive physical examinations.
• Develop, document, and communicate individualized care plans, ensuring that follow-up measures are implemented for optimal patient health.
• Perform daily rounds to monitor patient progress, adjusting care plans as necessary based on clinical assessments and laboratory findings.
• Prescribe medications, request laboratory and diagnostic tests, and interpret the results to inform clinical decisions.
• Coordinate care by communicating effectively with physicians, nursing staff, and other healthcare providers to meet patient needs.
• Create clear and precise discharge summaries, prescriptions, and referrals as part of patient discharge processes.
• Deliver post-discharge care instructions to patients, ensuring they understand their follow-up care and resources available to them.
• Engage actively in interdisciplinary team meetings to enhance patient care strategies while performing related duties as required.

Patient Population / Client Focus

Qualifications

• A Master’s Degree in a Nurse Practitioner Program is required, along with national board certification in your specialty area.
• A current, valid license to practice as a Nurse Practitioner and Registered Professional Nurse in New York State is mandatory.
• Experience cultivating collaborative agreements and attesting to professional relationships in accordance with New York's Nurse Practitioner Modernization Act (2015).

Preferred Experience

Candidates with a minimum of one year of clinical experience in women's health care are preferred. Strong interpersonal and communication skills are essential, as is a demonstrable commitment to patient-centered care. Familiarity with electronic health record systems, such as EPIC or Cerner, will be advantageous. Previous experience with interdisciplinary collaboration in a hospital setting will also be considered a plus.
