Camp Health Laboratory and Diagnostic Center is seeking a dedicated Internal Medicine Nurse Practitioner to join our team in Smithtown, New York. This full-time position, based onsite, offers a unique opportunity to provide high-quality primary care to adults on Medicare, ensuring they remain healthy and out of the hospital. Our Nurse Practitioners are essential in delivering compassionate, patient-centered care, engaging with patients independently, and collaborating with physicians to build trust and understanding in each patient's health journey.

Work Environment

As part of a dynamic care team, you will work collaboratively within a multidisciplinary environment that includes physicians, nurse practitioners, physician assistants, medical assistants, registered nurses, and care managers. This supportive structure enables you to focus on providing outstanding care for your patients, with the backing of a dedicated medical scribe to assist with documentation and administrative tasks. Our office operates Monday through Friday, from 8:00 AM to 5:00 PM, providing a structured work-life balance.

Key Responsibilities

• Provide exceptional primary care services, including routine and acute office visits.
• Conduct comprehensive annual wellness visits and health risk assessments, ensuring thorough documentation and a holistic view of patient health.
• Coordinate care with other providers, specialists, and testing facilities to streamline patient treatment.
• Lead population health initiatives to improve health outcomes, such as implementing evidence-based screening processes for eligible patients.
• Assist the care team by engaging in phone triage and patient outreach activities.
• Educate patients on their health conditions, treatment options, and care plans, fostering a strong understanding and connection to their healthcare journey.
• Participate in community outreach and promotional activities as needed.
• Conduct home visits when necessary to deliver care in a familiar setting for patients.
• Report to the Medical Director, working closely with operational leadership to enhance practice performance.

Patient Population / Client Focus

Your primary focus will be on older adults enrolled in Medicare, with a commitment to keeping them healthy and engaged in their care. This position allows you to play a crucial role in building long-term relationships with patients, understanding their individual health needs, and utilizing a value-based approach to care. By concentrating on comprehensive, community-centered health solutions, you will help patients live fulfilling lives, free from unnecessary hospital visits.

Qualifications

• Master of Science in Nursing, Doctor of Nursing Practice, or completion of an accredited Nurse Practitioner Certificate Program.
• National certification as a Family, Adult, or Adult-Gerontology Primary Care Nurse Practitioner through AANP or ANCC.
• Active, non-probationary state Nurse Practitioner license.
• Active DEA license for prescribing medications.
• A strong background in primary care settings, with experience specific to internal medicine, geriatrics, or family medicine.

Preferred Experience

• Familiarity with HCC (Hierarchical Condition Categories) documentation and ICD-10 coding.
• Proficiency in conducting Health Risk Assessments (HRAs).
• A passion for working in a collaborative care environment, showing a desire to innovate and improve patient outcomes.
• Bilingual skills are a plus, enhancing communication with our diverse patient population.

Compensation and Benefits

The Nurse Practitioner role at Camp Health Laboratory and Diagnostic Center offers a competitive salary ranging from $115,000 to $150,000 annually, commensurate with experience and expertise. Benefits include:
