Caring Connections Healthcare Services, LLC is actively seeking a dedicated and skilled Nurse Practitioner (NP) to join our team in providing comprehensive health services within a school setting in Smithtown, New York. This full-time, onsite position offers a competitive salary range of USD 150,000 to 160,000 annually. The successful candidate will be instrumental in addressing both acute and chronic health issues for our pediatric population while fostering collaborative relationships with healthcare providers, school personnel, and families.

Work Environment

Key Responsibilities

• Order and interpret diagnostic tests for accurate assessment of student health needs.
• Identify, manage, and provide treatment for common health issues, both acute and chronic, in accordance with established pediatric health guidelines.
• Develop and implement personalized care plans for students that may include medications, nutritional guidance, physical activity recommendations, and traditional nursing interventions.
• Work collaboratively with a multidisciplinary team to coordinate comprehensive patient care and ensure seamless referrals to specialists when necessary.
• Regularly evaluate and document patients' responses to treatment plans, adjusting as necessary to promote optimal health outcomes.
• Provide remote consultations, conduct thorough follow-ups, and triage walk-in patients to guarantee timely and effective care.
• Diagnose health conditions based on clinical findings and appropriately prescribe treatment as indicated.
• Serve as a resource for both healthcare staff and non-medically trained personnel, offering guidance on health issues as they arise.
• Engage in the mentoring and professional development of newly hired nurse practitioners to enhance team capabilities.
• Contribute to the development, review, and revision of practice guidelines specific to the needs of our student population.
• Conduct peer reviews to maintain high standards of care and support quality assurance initiatives.
• Uphold confidentiality and sensitivity concerning all patient interactions and documentation.
• Deliver direct patient care through scheduled clinical sessions, ensuring that every visit is thorough and compassionate.
• Perform health histories and problem-specific examinations tailored to the presenting concerns of students.

Patient Population / Client Focus

This position primarily serves the pediatric population within the school setting, where you will encounter a diverse range of health issues that impact children’s well-being and academic performance. You will be responsible for addressing physical health (e.g., illnesses, injuries), mental health concerns, and overall wellness, providing you with an opportunity to make a significant difference in the lives of students and their families.

Qualifications

• Active New York State Nurse Practitioner license and certification in Family Health or Adult Health is required; a Registered Nurse license is preferred.
• Current NPI number and DEA certification.
• A Master’s Degree in Nursing is a must, underscoring your advanced training and knowledge in this field.
• Ambulatory care experience is an advantage, showcasing familiarity with a variety of patient conditions and care strategies.
• A minimum of three years of relevant experience as a Nurse Practitioner, demonstrating your expertise in delivering high-quality healthcare services.
