Key Responsibilities

• Conduct comprehensive health assessments by obtaining detailed health histories and performing thorough physical examinations.
• Prescribe and oversee pharmacologic and non-pharmacologic treatment regimens tailored to individuals with chronic and acute pain.
• Instruct patients and their families on pain management strategies, ensuring they understand treatment goals, compliance with medication regimens, and potential side effects.
• Manage care for patients struggling with chronic pain conditions, utilizing evidence-based approaches.
• Prescribe appropriate medications and maintain a check on the Prescription Monitoring Program (PMP) for safety in prescribing practices.
• Collaborate with interdisciplinary teams, consulting with physicians and specialists to craft personalized treatment plans for complex cases.
• Utilize strong clinical assessment and diagnostic skills to evaluate and attend to patients' needs effectively.
• Develop and implement individualized pain management plans that incorporate patient preferences and treatment goals.

Patient Population / Client Focus

You will primarily focus on residents at the assisted living facility who present with a wide range of pain disorders. Qualifications

• A Master's Degree in Nursing (MSN) is required.
• An active and valid New York State Nurse Practitioner license is mandatory.
