Work Environment

This role is onsite at our Assisted Living Facility, where our PNPs collaborate closely with a multidisciplinary team to deliver the highest standard of care. The work setting is designed to be nurturing and supportive, allowing healthcare professionals to thrive while fostering a healthy environment for families. Standard working hours are Monday to Friday, from 9 a.m. to 5 p.m., ensuring a work-life balance that our team members highly value.

Key Responsibilities

• Deliver comprehensive healthcare to patients, focusing on well-child exams, preventive care, and the management of acute and chronic pediatric conditions.
• Conduct thorough assessments of physical, psychological, and social health, tailoring care strategies that meet the specific needs of each patient.
• Implement therapeutic procedures including immunizations, injections, suturing, wound care, and prompt management of infections.
• Collaborate with physicians to develop and implement effective patient management plans, maintaining accurate and up-to-date progress notes.
• Review medical charts for quality assurance and facilitate follow-up care, ensuring continuity in patient treatment.
• Educate families on health maintenance, developmental milestones, and behavioral health resources, fostering proactive healthcare habits.
• Regularly participate in team meetings to discuss patient care strategies and quality improvement initiatives.

Patient Population / Client Focus

This role specifically caters to a diverse pediatric population, from newborns to young adults, addressing a wide array of health concerns including developmental disorders, common illnesses, and chronic conditions. Your expertise will be crucial in recognizing and managing the unique physical, emotional, and social challenges faced by children, thereby significantly contributing to their overall health outcomes.

Qualifications

• Completion of an accredited Nurse Practitioner program, granting you certification as a Nurse Practitioner.
• An advanced nursing degree (MSN or DNP) along with current RN and NP licensure in New York.
• A minimum of 2 years of relevant pediatric experience, with some preference for candidates who demonstrate substantial exposure to similar settings.
• Current certifications from recognized boards including the American Nurses Credentialing Center or equivalent.
• Basic Life Support (BLS) and Advanced Cardiac Life Support (ACLS) certifications are mandatory.
• Exceptional communication and interpersonal skills with a strong ability to work collaboratively within a team.

Preferred Experience

• Experience in an assisted living or similar healthcare setting.
• Familiarity with electronic health record (EHR) systems, particularly those used in pediatrics.
• Previous involvement in quality improvement activities related to pediatric care.
