Work Environment

This role is based onsite at our assisted living facility in Smithtown, where you will work closely with a team of healthcare professionals to deliver high-level care to our residents. The facility is equipped with advanced electronic medical record (EMR) systems, fostering a collaborative atmosphere for providing patient-centered care. As an NP, you will have the opportunity to interact with residents on a daily basis, ensuring they receive personalized treatment plans tailored to their individual needs.

Key Responsibilities

• Develop and implement comprehensive treatment plans, effectively communicating them to residents, families, and interdisciplinary care teams.
• Order and interpret diagnostic testing, adjusting treatment plans based on results to provide the most effective care.
• Maintain accurate and compliant documentation in our EMR system, ensuring continuity and quality of care.
• Collaborate with local medical staff and leadership to align care strategies with organizational goals, including value-based care initiatives.
• Deliver exceptional customer service to ensure high satisfaction levels among residents and their families.
• Practice in accordance with professional standards, ethical guidelines, and state regulations.
• Coordinate care with local primary care physicians (PCPs) and various specialists, including telemedicine providers when necessary.
• Participate in regulatory visits and conduct health assessments to ensure compliance and quality of care.
• Engage in family meetings to discuss care plans and provide education about advanced care planning.
• Address pharmacy-related inquiries and recommendations, enhancing medication management for residents.
• Ensure adequate onsite presence during working hours to meet the needs of residents and facility standards.
• Assess, diagnose, and manage both chronic and acute health conditions while providing ongoing health education to residents.

Patient Population / Client Focus

This position primarily serves the senior population residing within our assisted living facility in Smithtown, New York. As an NP, you will be expected to exhibit a genuine passion for delivering care to elderly individuals, many of whom may have complex medical histories or require supportive services tailored to their unique needs. Your expertise will play a vital role in improving their health outcomes and overall well-being.

Qualifications

• National certification from the American Association of Nurse Practitioners (AANP) or the American Nurses Credentialing Center (ANCC).
• Active and unrestricted state license to practice as a Nurse Practitioner in New York.
• Current DEA license.
• Authorization to work in the United States.
• Ability to adapt to varying schedules and treatment modalities.

Preferred Experience

• A Master of Science in Nursing (MSN) or a Doctor of Nursing Practice (DNP) is highly desirable.
• Experience working with senior populations and in assisted living settings will be preferred.
• Familiarity with electronic medical record systems and experience in team-based healthcare delivery is beneficial.
• A strong passion for serving low-income and underserved populations.

Compensation and Benefits
