Advanced Cardiac Care Nurse Practitioner (NP) - Full-Time

Position Overview

The Gundersen Health and Wellness Clinic is actively seeking an experienced Nurse Practitioner (NP) to join our dedicated team at the Center for Advanced Cardiac Care (CACC) in East Islip, New York. This full-time role offers the opportunity to work within an outpatient setting, collaborating with a multidisciplinary team to provide exceptional care to patients with a variety of cardiac conditions. As an integral member of the healthcare team, the Nurse Practitioner will report directly to the Chief Nurse Practitioner and work closely under the guidance of the CACC Medical Director.

Work Environment

The Nurse Practitioner will operate in a dynamic onsite environment within an Assisted Living Facility, where the focus is on providing tailored healthcare solutions to patients who may face mobility or access issues. The NP will interact daily with a supportive administrative team, skilled healthcare professionals, and patients, contributing to a cohesive care environment that prioritizes the wellbeing of every individual.

Key Responsibilities

As a Nurse Practitioner at CACC, your day-to-day responsibilities will include:

• Conducting patient assessments by obtaining detailed medical histories and performing comprehensive physical examinations tailored to cardiac health.
• Ordering and interpreting diagnostic tests like echocardiograms or stress tests, diagnosing both acute and chronic cardiac conditions.
• Developing and managing individualized patient care plans in collaboration with physicians and other healthcare providers that encompass preventive measures and treatment strategies.
• Educating patients and their families about their medical conditions, treatment options, and health maintenance, empowering them to take an active role in their healthcare.
• Coordinating care with a wide range of specialists, including social workers, nutritionists, and pharmacists, to create a comprehensive approach to patient management.
• Documenting all patient interactions meticulously within our electronic medical records (EMR) system, ensuring accurate and up-to-date patient information is always available.
• Actively participating in quality improvement initiatives and evidence-based research to help enhance clinical practices and patient outcomes.
• Observing and adhering to all safety protocols, fostering a supportive and safe environment for both staff and patients.

Patient Population / Client Focus

The primary patient population consists of individuals with diverse cardiac issues, including but not limited to heart failure, pulmonary arterial hypertension, and patients undergoing advanced therapies such as outpatient intravenous medication or mechanical circulatory support. The facility ensures holistic care that addresses not only the physical but also the emotional and social aspects of patients' health.

Qualifications

To be considered for this role, candidates must possess the following:

• A Master’s degree in Nursing, with specialized training as a Nurse Practitioner in adult care.
• Current New York State licenses as a Registered Professional Nurse and Nurse Practitioner.
• Certification as a Nurse Practitioner (NP Board certification) and current Basic Life Support (BCLS) and Advanced Cardiovascular Life Support (ACLS) certifications.
• A solid foundation in using electronic medical records and proficiency with standard software applications like MS Word and MS Excel.

Preferred Experience

Ideal candidates will have at least 2 years of experience in a hospital or outpatient setting, preferably in cardiology. Familiarity with advanced cardiac conditions and experience in managing diverse patient populations is highly desirable. Candidates who have experience with heart transplant patients or advanced therapies will be given preference.

Compensation and Benefits

The position offers a competitive salary range of USD 120,000 to 220,000 per year, commensurate with experience and qualifications. In addition, we provide a comprehensive benefits package that includes:
