Work Environment

The Nurse Practitioner will work on-site in patients' homes, providing care tailored to the unique needs of our clients. Key Responsibilities

• Conduct comprehensive health assessments, developing individualized care plans that meet the medical and emotional needs of patients.
• Monitor and manage chronic and acute health conditions, adjusting treatment plans as necessary to enhance patient health outcomes.
• Maintain careful documentation of patient progress notes and nursing care plans using electronic health record (EHR) systems to ensure continuity of care.
• Conduct patient education sessions to empower patients and their families with health knowledge and self-management strategies.
• Participate in quality improvement initiatives, assessing current practices, and recommending changes to improve service delivery and patient satisfaction.
• Collaborate with interdisciplinary teams to coordinate comprehensive care and ensure patients’ rights and obligations are upheld.
• Engage in professional development by attending workshops and conferences to stay abreast of the latest advancements in nursing practice.

Patient Population / Client Focus

The Nurse Practitioner will work with a diverse patient population, including elderly clients with chronic conditions, individuals recovering from surgery, and patients seeking preventive health services. The NP’s primary focus will be on holistic, patient-centered care that respects each individual's values and needs, ensuring every client receives tailored support that fosters their overall well-being.

Qualifications

• Master of Science in Nursing (MSN), Doctor of Nursing Practice (DNP), or completion of a Nurse Practitioner Certificate Program.
• National certification from either the American Association of Nurse Practitioners (AANP) or the American Nurses Credentialing Center (ANCC).
• Active and unrestricted Nurse Practitioner license in the State of New York.
• Authorization to work in the United States.
• Exceptional communication and interpersonal skills, with a strong commitment to a team-oriented approach.

Preferred Experience

• Previous experience in home care, geriatric care, or a similar field is strongly desired.

Compensation and Benefits

Comfort of Care Healthcare Services offers a competitive salary range of USD 150,000 to 160,000 per year, based on caseload and experience.
