Parkland Health and Hospital PC is seeking dedicated and compassionate Nurse Practitioners to join our team in providing high-quality primary care in Babylon, New York. This full-time position involves working closely with patients in a home care setting, where you will be responsible for diagnosing and treating various health conditions while promoting optimal health practices.

Work Environment

This position is primarily onsite, allowing for intimate interactions with patients in their homes. You will be an integral member of a dynamic healthcare team that emphasizes collaboration and communication in a primary care context. Working hours are Monday to Friday, 40 hours a week, and the practice typically concludes by 5:00 PM. This enables a balanced professional and personal life, contributing to employee satisfaction.

Key Responsibilities

• Conduct thorough physical examinations to assess patient health.
• Order and interpret diagnostic tests, such as blood tests and X-rays, to aid in accurate diagnoses.
• Diagnose a wide range of health conditions, both chronic and acute.
• Develop and propose evidence-based treatment plans for managing patient care.
• Prescribe necessary medications and monitor their effects on patients effectively.
• Maintain meticulous patient records and manage schedules to ensure continuity of care.
• Communicate health conditions and treatment plans clearly to patients and their families.
• Educate patients about disease prevention and healthy lifestyle choices.
• Collaborate with other healthcare professionals to provide holistic patient care.
• Engage in ongoing education and research to continually enhance your clinical knowledge and expertise.

Patient Population / Client Focus

The Nurse Practitioner will have the opportunity to work with a diverse patient population, focusing on individuals requiring home-based care. You will be integral in managing not only routine health assessments but also addressing chronic health conditions, ensuring that patients receive the guidance and support necessary for improved health outcomes.

Qualifications

• A valid Nurse Practitioner license in New York State.
• A Master's degree in Nursing or a relevant Nurse Practitioner program.
• Certification as an Adult or Family Nurse Practitioner (ANP/FNP).
• A DEA number for prescribing controlled substances.
• Proven experience working in clinical settings such as hospitals, clinics, or urgent care facilities.
• Strong understanding of health regulations and patient care standards.
• Exceptional communication and interpersonal skills.
• A commitment to ethical principles in healthcare practices.

Preferred Experience

• Experience in primary care settings and knowledge of care models tailored for home care.
• Familiarity with patient management software and electronic health record systems.
• Fluency in additional languages is a plus but not required, as it may enhance patient communication.

Compensation and Benefits

The position offers a competitive salary ranging from USD 140,000 to 180,000 per year, depending on your experience and qualifications. In addition to a favorable salary, we provide a comprehensive benefits package that includes:
