Work Environment

This is an onsite position, where you will be situated within various home care environments across Huntington Station and surrounding areas. Key Responsibilities

As a Nurse Practitioner at Expert Cares, your responsibilities will include but are not limited to:

• Providing comprehensive health education to residents and caregivers about chronic and acute conditions, as well as preventive health strategies.
• Developing individualized treatment plans that are effectively communicated to all relevant stakeholders, ensuring clarity in the care process.
• Ordering and interpreting medical tests, adjusting treatment plans based on diagnostic outcomes, and collaborating with other healthcare professionals.
• Maintaining accurate and compliant electronic medical records (EMR) to document patient encounters and treatment plans.
• Leading initiatives that align with both local and regional operational goals, thus supporting strategic directions related to value-based care.
• Ensuring a customer-oriented experience that prioritizes resident satisfaction and retention through compassionate interactions.
• Adhering to ethical standards and complying with state and federal regulations in every aspect of care.
• Working closely with Primary Care Physicians (PCPs) and other healthcare team members to coordinate care effectively.
• Completing required regulatory visits and acute care rounds in a timely manner, addressing the needs of residents as they arise.
• Facilitating family conferences to discuss advanced care planning and educational aspects related to resident health.
• Engaging with pharmacy professionals regarding medication management and recommendations.
• Being present in assigned facilities during operational hours to ensure sufficient face-to-face coverage for resident needs.
• Assessing, diagnosing, and managing a wide range of health conditions, ensuring that both acute and chronic issues are addressed meticulously.

Patient Population / Client Focus

The primary focus of your care will be on seniors and low-income populations who require medical attention in home care environments. Qualifications

To be considered for this role, candidates must meet the following qualifications:

• National certification from the American Association of Nurse Practitioners (AANP) or the American Nurses Credentialing Center (ANCC).
• An active, unrestricted state nursing license in New York.
• An active DEA license for prescribing medications.
• Legal authorization to work in the United States.
• A Master's Degree in Nursing, Doctor of Nursing Practice, or completion of an accredited Nurse Practitioner certificate program is required.

Preferred Experience

While not mandatory, experience providing care in home healthcare settings or working directly with geriatric populations is preferred. Additionally, proficiency in electronic medical record systems and familiarity with value-based care models will be beneficial.

Compensation and Benefits
