Full-Time Nurse Practitioner in Home Care Services

Work Environment

This role will be performed onsite, primarily within clients' homes, allowing for direct interaction with patients in their familiar surroundings. The work setting promotes a relationship-based approach to care, ensuring that NPs can build trust and rapport with residents. You will be part of a dynamic team that collaborates with local healthcare providers and specialists, ensuring comprehensive care delivery.

Key Responsibilities

• Conduct thorough health assessments and obtain detailed medical histories from patients to inform care decisions.
• Develop individualized treatment plans in collaboration with residents, their families, and the multidisciplinary care team.
• Order and interpret lab tests and diagnostic imaging to monitor health status and adapt treatment protocols as necessary.
• Utilize electronic medical record (EMR) systems to maintain accurate and compliant documentation of patient encounters.
• Participate in case conferences and facilitate advanced care planning discussions to align resident care with their goals and preferences.
• Collaborate with primary care physicians (PCPs) and other specialists to coordinate care effectively, including telemedicine options when appropriate.
• Ensure regulatory compliance by completing all required visits as mandated by state and federal guidelines.
• Provide education on health promotion and disease prevention practices to residents and their families.
• Be present on-site during designated hours to guarantee that there is adequate face-to-face coverage for residents' care needs.
• Take part in continuous quality improvement initiatives and support the organization’s strategic focus on value-based care.

Patient Population / Client Focus

This position primarily serves seniors and individuals residing in home care settings who may have complex health needs. You will engage with patients from diverse backgrounds, with a significant emphasis on providing empathetic care to low-income populations. Your role will involve actively participating in their healthcare journey, ensuring they receive the attention, resources, and services necessary for optimal health outcomes.

Qualifications

• Master of Science in Nursing, Doctor of Nursing Practice, or completion of an NP Certificate Program.
• Current national certification from AANP or ANCC.
• Active, unrestricted state license to practice as a Nurse Practitioner in New York.
• Active DEA license for prescribing medications.
• Authorization to work in the United States.
• Ability to adapt to a flexible schedule and work environment.
• Genuine passion for providing healthcare to seniors and underserved populations.

Preferred Experience

• Experience in home care, geriatrics, or a related field is highly desirable.
• Familiarity with various EMR systems and telehealth platforms.

Compensation and Benefits

Health Partners Wellness Solution, LLC offers a competitive salary range of USD 150,000 – 160,000 per year, commensurate with experience and qualifications. We provide a comprehensive benefits package that includes:
