Work Environment

This role operates onsite, delivering care directly in the home care setting. By working closely with patients and their families, our Nurse Practitioner will navigate the complexities of home healthcare, addressing both medical and emotional needs. Key Responsibilities

• Conduct comprehensive assessments by performing thorough surgical, medical, and psychosocial health histories and examinations, documenting critical findings to initiate and maintain an up-to-date problem and medication list.
• Order, interpret, and follow up on diagnostic tests, including lab work and imaging studies, to formulate clinical impressions and adjust treatment plans based on evidence and observed patient responses.
• Facilitate smooth hospital admissions, transfers, and discharges, ensuring that appropriate arrangements for inpatient and outpatient care are in place, as well as coordinating home care services for ongoing support.
• Lead patient care activities, establishing priorities for patient teaching, counseling, and discharge planning, while effectively delegating tasks to other healthcare professionals when appropriate.
• Perform phone triage during acute clinical situations, engaging in assessments as needed and prescribing treatments in accordance with established protocols.
• Contribute to the development and evaluation of standards of care, collaborating with healthcare team members to align practices with institutional policies and regulatory agency requirements.
• Engage in related duties and responsibilities as required to meet team needs and enhance service delivery.

Qualifications

• Current New York State Registered Nurse (RN) License.

• Current DEA License.
• BLS (Basic Life Support) and ACLS (Advanced Cardiovascular Life Support) certification.
• Proficiency in computer systems with an ability to quickly adapt to new healthcare technologies and electronic medical records.
• Strong organizational skills and attention to detail, ensuring precise documentation and adherence to care protocols.
• Maintaining and renewing your New York State Nurse Practitioner License as required is essential for this position.

Preferred Experience
