Gotham Enterprises Ltd is seeking a dedicated Lead Nurse Practitioner to join our Center for Advanced Cardiac Care (CACC) in Glen Cove, New York. This full-time, onsite role will involve comprehensive patient care within our home care facility. The successful candidate will work under the guidance of the Chief Nurse Practitioner and collaborate closely with the CACC Medical Director. Work Environment

This role is embedded within a dynamic outpatient practice dedicated to cardiology, where you will interact with a diverse team composed of physicians, nurse practitioner colleagues, registered nurses, and various healthcare professionals, including social workers and nutritionists. Key Responsibilities

• Conduct thorough patient assessments by obtaining detailed medical histories and performing comprehensive physical examinations to inform clinical decisions.
• Order and interpret diagnostic tests while managing an array of chronic and acute conditions prevalent among our cardiac patient demographic.
• Formulate and execute individualized care plans, ensuring they align with established protocols and incorporate health maintenance strategies, anticipatory guidance, and necessary interventions.
• Facilitate coordination of care with inpatient teams through safe hospital admissions and effective discharge planning.
• Engage in patient education, counseling patients and their families on management options and lifestyle choices that support cardiac health outside of hospital settings.
• Maintain accurate and detailed documentation of all patient interactions within our electronic medical record (EMR) system, ensuring continuity and quality of care.
• Collaborate with multidisciplinary teams to enhance patient outcomes concerning advanced therapies, ensuring that all transitions are smooth and well-coordinated.
• Involve yourself in research initiatives aimed at improving clinical practices and treatment protocols based on the latest evidence-based findings.
• Uphold accountability for nursing practices and adhere to safety standards and ethical guidelines throughout your patient interactions.
• Assist with additional responsibilities as assigned to support team and clinic operations.

Patient Population / Client Focus

The Nurse Practitioner will encounter a varied population focusing primarily on patients suffering from complex cardiac conditions, including but not limited to advanced heart failure, pulmonary arterial hypertension, and post-heart transplant management. Familiarity with outpatient medication support and mechanical circulatory devices will be essential for effectively managing these patients.

Qualifications

• A Master’s degree in Nursing with specialization as a Nurse Practitioner, coupled with at least two years of direct experience in a clinical setting, preferably focused on cardiology.
• Possession of current New York State Registered Professional Nurse and Nurse Practitioner licenses, along with NP Board certification.
• Certifications in BLS, ACLS, DEA, and NPI are required.
• Proficient using computerized systems, including electronic medical records software and standard office applications such as MS Office.

Preferred Experience

A background in caring for patients with advanced heart failure and a solid understanding of outpatient cardiac therapies such as intravenous medication support or mechanical circulatory support will be advantageous. Experience in a collaborative medical team, particularly in fast-paced environments typical of cardiology, is also desirable.

Compensation and Benefits

We offer a competitive salary, ranging from USD 120,000 to 220,000 per year, determined by experience and qualifications.
