Work Environment

As a Nurse Practitioner at Graceful Care Clinic, you will work primarily in an onsite home care setting, allowing for personalized healthcare delivery in the comfort of patients' homes. Your work schedule will typically be from 9:00 a.m. to 6:00 p.m., with occasional evening clinics extending until 8:00 p.m. Additionally, participation in community outreach events and health fairs during weekends may be required, enabling you to engage directly with the community and enhance public health awareness.

Key Responsibilities

• Conduct thorough health assessments and screenings for both individuals and families, ensuring a holistic understanding of their health status.
• Diagnose and manage acute and chronic medical conditions, focusing on preventive care and promoting healthy lifestyles.
• Collaborate with local community organizations to develop and implement impactful health education programs that cater to diverse populations.
• Provide culturally sensitive care, advocating for patients' rights while addressing systemic barriers to healthcare access.
• Coordinate referrals and follow-up care, ensuring patients receive all necessary services for continuity of care.
• Maintain meticulous and accurate medical records in compliance with regulatory standards, utilizing electronic health record systems effectively.

Qualifications

• Current certification as a Nurse Practitioner is required.
• Active state nursing license in New York.
• A Master’s degree in nursing from an accredited institution.
• A minimum of 3 years of clinical experience, ideally in community health, public health, or similar settings.
• Exceptional interpersonal and communication skills, essential for effective collaboration with patients and community partners.
• A demonstrated commitment to health equity and advocacy for vulnerable populations.

Preferred Experience

Compensation and Benefits

The salary for this position ranges from $180,000 to $200,000 annually, commensurate with your experience and qualifications.
