Key Responsibilities

• Conduct comprehensive cardiovascular assessments and screenings, utilizing tools and technologies to evaluate heart health and detect abnormalities.
• Diagnose and manage a range of acute and chronic heart conditions, adapting treatment plans to reflect individual patient needs and preferences.
• Prescribe medications as needed, and educate patients on their use, potential side effects, and lifestyle modifications that promote heart health.
• Create and implement tailored health promotion and disease prevention strategies, focusing on enhancing the cardiovascular health of the community.
• Maintain precise and compliant medical records reflecting patient care activities, treatment plans, and outcomes in accordance with regulatory standards.
• Collaborate with local community organizations to develop and deliver educational programs that raise awareness about heart disease and prevention strategies.

Patient Population / Client Focus

You will work with a diverse patient population that includes individuals of varying ages and backgrounds, with a particular emphasis on those at risk for or living with cardiovascular diseases. Your culturally sensitive approach will ensure that all patients receive care that acknowledges their unique needs and circumstances. Building strong relationships with your patients will be key in fostering trust and encouraging adherence to treatment plans.

Qualifications

• A Master’s degree in Nursing from an accredited program.
• Current and active state nursing license with certification or credentials in Cardiovascular Nursing or a related specialty.
• Strong foundational knowledge in cardiovascular health along with clinical experience demonstrating high levels of competency in patient evaluation and care.

Preferred Experience

• 3 to 5 years of clinical experience in cardiology or cardiovascular health settings, with a focus on community or public health being highly advantageous.
• Previous roles in patient education or health promotion initiatives related to cardiovascular care.
• Comfort with data management systems and electronic health records (EHR) for maintaining patient documentation and collaborating with healthcare teams.

Compensation and Benefits
