Work Environment

This position is primarily onsite, where you will conduct home visits to provide care. Your work schedule will typically range from 9:00 a.m. to 6:00 p.m., with occasional evening clinics running until 8:00 p.m. Participation in community outreach initiatives and health fairs may also be required on weekends, allowing for flexible engagement with the community. You can expect an average workweek of around 40 hours, focusing on impactful interactions with patients.

Key Responsibilities

• Perform comprehensive health assessments and screenings in patients' homes, ensuring individualized care.
• Diagnose and manage both acute and chronic conditions, maintaining a focus on prevention and health promotion strategies.
• Create and implement tailored health education programs in collaboration with local community organizations, addressing specific health concerns and improving health literacy.
• Provide culturally competent care by actively understanding the diverse backgrounds of patients and tailoring your approach to meet their specific needs.
• Advocate for patients' rights and access to healthcare, ensuring that underserved populations receive the care they need.
• Coordinate referrals to specialists and arrange follow-up care, maintaining continuity of care for all patients.
• Document all patient interactions accurately in compliance with healthcare regulations, safeguarding patient confidentiality and continuity of care.
• Actively participate in community health outreach programs and health fairs, offering screenings and educational sessions to improve health outcomes.
• Stay informed about community health trends, embracing evidence-based practices and policies that impact patient care.

Patient Population / Client Focus

Your primary focus will be on serving diverse populations within the Babylon community, including individuals experiencing socioeconomic challenges and those with limited access to traditional healthcare. You will be instrumental in understanding their unique healthcare needs—working collaboratively to address and navigate various barriers to care.

Qualifications

• Current certification as a Nurse Practitioner, demonstrating competency in advanced nursing practices.
• An active state nursing license, ensuring compliance with New York state regulations.
• A Master's degree in nursing from an accredited institution, reflecting a robust understanding of nursing science and practice.
• A minimum of 3 years of clinical experience, with a preference for those with backgrounds in community health or public health settings.
• Exceptional interpersonal and communication skills, capable of building rapport with diverse patient populations and working effectively within a multidisciplinary team.
• A strong commitment to social justice, health equity, and proactive advocacy for vulnerable communities.

Preferred Experience

• Experience in home healthcare or community-based care models, which will enhance understanding of the complexities and needs of home care patients.
• Demonstrated ability to develop and implement health education initiatives tailored to specific community health issues.
• Familiarity with electronic health records (EHR) systems to ensure accurate documentation and efficient workflow.

Compensation and Benefits
