Work Environment

The work schedule is typically from Monday to Friday, with flexible hours of either 9 AM to 5 PM or 8 AM to 4 PM, allowing for work-life balance. Key Responsibilities

• Conduct thorough assessments, including diagnosing and managing acute and chronic illnesses in patients.
• Prescribe appropriate medications and treatments based on clinical guidelines and patient needs.
• Perform comprehensive physical examinations to monitor health status and detect any issues early.
• Collaborate with physicians and specialists to develop, implement, and direct patient management policies.
• Facilitate referrals as necessary, ensuring seamless transitions for patients requiring specialized care.
• Maintain accurate and updated medical records, including detailed patient histories and care plans.
• Educate patients and their families about health maintenance, disease prevention, and self-care strategies.
• Participate in regular team meetings and training sessions to enhance practice protocols and patient care processes.

Patient Population / Client Focus

In this role, you will primarily work with elderly patients in an assisted living setting, addressing a range of urgent care needs. Your expertise will assist patients dealing with acute medical conditions, managing chronic diseases, and understanding the complexities of aging-related health issues. You will also play a vital role in health education, providing patients with the knowledge needed to make informed decisions about their health.

Qualifications

• Master's degree in Nursing with a specialization as a Nurse Practitioner (NP) is required.
• Active state Nurse Practitioner license in New York is mandatory.
• At least 2 years of clinical experience in urgent care or a related field.
• Nationally recognized certification as a Nurse Practitioner with prescriptive authority.
• Strong knowledge of medical terminology and pharmacological principles.

Compensation and Benefits
