Full-Time Nurse Practitioner - Outpatient Care Specialist

The Elderly Care & Rehab Clinic, located in Huntington Station, New York, is seeking a dedicated and compassionate Nurse Practitioner (NP) to join our full-time team. The ideal candidate will play an essential role in providing high-quality primary care to our Medicare-aged patients in an outpatient setting, ensuring that they receive exceptional treatment while promoting their well-being and independence.

Work Environment

This role operates within our outpatient office located in Huntington Station, NY, and requires onsite presence during regular business hours. The work schedule is Monday to Friday, from 9:00 AM to 5:00 PM. You will be part of a close-knit healthcare team that emphasizes interprofessional collaboration, utilizing a dedicated medical scribe, medical assistants, and registered nurses to streamline care delivery for our patients.

Key Responsibilities

• Provide comprehensive primary care by conducting thorough assessments and creating individualized care plans tailored to the unique medical needs of each patient.
• Perform office visits to address acute issues, manage chronic conditions, and follow up on patient progress while maintaining meticulous and thoughtful documentation.
• Facilitate annual wellness visits and health risk assessments, ensuring a holistic approach to each patient’s health as well as thorough documentation to optimize coding and billing accuracy.
• Coordinate with a multidisciplinary care team, which includes specialists and community services, to ensure well-rounded and continuous care for patients.
• Lead population health initiatives, such as ensuring eligible patients receive necessary screenings, including evidence-based breast cancer checks every two years.
• Provide patient education regarding care plans, treatments, and health conditions, fostering a trusting relationship that encourages patient engagement in their health journey.
• Assist the care team with phone triage and outreach initiatives to enhance the patient experience and ensure timely responses to patient needs.
• Participate in promotional activities that align with the mission and vision of the clinic.
• Conduct home visits as necessary, further supporting patients who may have difficulty accessing the clinic.
• Report directly to the Medical Director and engage closely with operational leadership to achieve organizational goals.

Patient Population / Client Focus

Your primary focus will be on elderly adults, particularly those enrolled in Medicare. Understanding the unique healthcare challenges faced by this population, you will deliver personalized care aimed at enhancing their quality of life, promoting preventive care measures, and addressing their specific health needs.

Qualifications

• A Master of Science in Nursing, Doctor of Nursing Practice, or completion of an accredited Nurse Practitioner certificate program.
• National certification as a Family, Adult, or Adult-Gerontology Primary Care Nurse Practitioner, obtained through AANP or ANCC.
• A valid and active nurse practitioner license in New York State without any probationary status.
• An active DEA license for prescribing controlled substances.
• Prior experience in primary care settings, including geriatrics, internal medicine, or family medicine.
• Familiarity with HCC documentation, ICD-10 coding, and health risk assessments to ensure comprehensive care and appropriate reimbursement.
• A passion for collaborative practice and the ability to thrive in a cross-functional team environment.

Preferred Experience

• Experience working specifically with the geriatric population, preferably in a primary care context.
• Proficiency in EMR systems and a solid understanding of medical record documentation standards.
• Skills in patient education and community outreach.

Compensation and Benefits

The annual salary for this full-time Nurse Practitioner role ranges from USD 120,000 to 160,000, commensurate with experience and qualifications.
