Key Responsibilities

• Develop and implement individualized patient care plans based on comprehensive assessments and evaluations.
• Monitor and record patients' health status, maintaining detailed progress notes and nursing care plans to ensure continuity of care.
• Conduct thorough patient education on health management, wellness strategies, and disease prevention to empower patients and their families.
• Facilitate communication between patients, families, and interdisciplinary team members, interpreting clinical findings and encouraging collaborative problem-solving.
• Participate in the enhancement of care strategies through ongoing evaluation of nursing practices and procedures.
• Stay updated on advancements in nursing and healthcare practices through continuing education, contributing to professional development initiatives.
• Fulfill any additional responsibilities as assigned by nursing service administration to support the successful delivery of care.

Patient Population / Client Focus

This position serves a diverse population, from pediatric to geriatric patients, many of whom are managing chronic illnesses or post-surgical recovery. Qualifications

• A Bachelor’s Degree in Nursing or a related field, accompanied by a Master of Science in Nursing, Doctor of Nursing Practice, or an accredited NP Certificate Program diploma.
• Current national certification from the American Association of Nurse Practitioners (AANP) or the American Nurses Credentialing Center (ANCC).
• An active and unrestricted New York state NP license.
• Authorization to work in the United States without restrictions.
• Strong adaptability to work in a home care environment with a flexible schedule.

Preferred Experience

Compensation and Benefits
