Work Environment

The Nurse Practitioner will operate in an onsite school setting, enhancing student health through direct patient engagement in a comfortable and familiar environment. Key Responsibilities

• Conduct thorough cardiovascular assessments on students, including detailed history taking and physical examinations, to identify and manage cardiovascular health issues effectively.
• Provide tailored patient education, focusing on heart health management, medication adherence, lifestyle modifications, and preventive strategies to empower students to take charge of their health.
• Monitor patient progress through regular follow-up visits, documenting changes in condition, and adjusting treatment plans as necessary to optimize outcomes.
• Collaborate with both inpatient and outpatient care services, participating in activities such as discharge planning and follow-up appointments, ensuring seamless transitions in care.

Qualifications

Compensation and Benefits
