Work Environment

In this onsite role, you will work directly within patients' homes, fostering relationships in a friendly, respectful, and caring environment. Your workday typically runs from 9:00 a.m. to 6:00 p.m., with the potential for additional evening clinics and weekend health fairs. The average workweek totals around 40 hours, allowing you to maintain a work-life balance while making a significant difference in your community.

Key Responsibilities

• Conduct thorough health assessments and screenings for individuals and families to develop personalized care plans.
• Diagnose and manage both acute and chronic medical conditions, offering preventive care and promoting overall health.
• Collaborate with local organizations to create and implement health education initiatives, targeting prevalent health issues in the community.
• Deliver culturally competent care and advocate for patients' rights, ensuring accessibility to essential healthcare resources.
• Coordinate referrals and follow-up care to ensure seamless continuity in patient care.
• Maintain comprehensive and precise medical records in accordance with all regulatory standards and internal policies.
• Actively participate in community outreach programs and health fairs, providing health screenings and resources to enhance public awareness.
• Stay informed on community health trends, evidence-based practices, and relevant healthcare policies to better serve your patients.

Patient Population / Client Focus

This role primarily focuses on serving diverse populations within Lynbrook and the surrounding areas. You will work closely with individuals and families from various backgrounds, ensuring that care is tailored to meet their unique cultural, social, and health needs. Your efforts will directly contribute to improving health outcomes and empowering residents to take control of their health.

Qualifications

• A Master's or Doctoral degree in Nursing from an accredited program.
• Certification as a Family Nurse Practitioner (FNP), Adult-Gerontology Nurse Practitioner (ANP), or Adult-Gerontology Primary Care Nurse Practitioner (AGNP) by either the American Nurses Credentialing Center (ANCC) or the American Academy of Nurse Practitioners (AANP).
• A current, unrestricted Nurse Practitioner (APRN) license in New York State.
• DEA# and NPI# are required for prescribing medication.
• Strong clinical assessment and diagnostic skills, with the ability to conduct comprehensive evaluations and make informed decisions about patient care.
• Excellent communication and interpersonal skills to engage effectively with patients and their families.
• Ability to work independently while collaboratively being part of a dedicated healthcare team.
• Proficiency in using electronic health records (EHR) systems to document patient information and improve care coordination.

Preferred Experience

• A minimum of 2 years of clinical experience, particularly in primary care, family medicine, or community health settings.
• Experience working with underserved populations and engaging in community health initiatives is highly valued.
• Strong organizational abilities and a proactive approach to patient care management.

Compensation and Benefits

The expected salary range for this position is USD 150,000 to 160,000 per year, commensurate with experience. Additionally, NextGen NP Solutions offers a comprehensive benefits package, including health insurance, retirement plans, and paid time off. A sign-on bonus of up to $10,000 is also available to help you transition into your new role.
