This full-time, onsite position focuses on providing outstanding primary care services to adults, primarily those on Medicare, with an annual salary range of USD 180,000 to 220,000, commensurate with experience.

Work Environment

The selected Adult Nurse Practitioner will work in a supportive, dynamic home care setting, collaborating with a multidisciplinary team that includes physicians, physician assistants, nurse practitioners, medical assistants, registered nurses, and care managers. Key Responsibilities

• Deliver exceptional primary care by conducting thorough assessments and developing tailored care plans for patients.
• Provide routine and acute care during office visits, ensuring comprehensive documentation and compliance with health regulations and standards.
• Administer annual wellness visits and health risk assessments, integrating a holistic perspective on individual health needs.
• Coordinate patient care by collaborating with other healthcare providers and specialists, overseeing referrals and follow-ups.
• Take the lead in population health initiatives by ensuring adherence to evidence-based clinical guidelines, such as breast cancer screenings.
• Support phone triage and outreach efforts to identify and address patient concerns proactively.
• Educate patients on their health conditions, care plans, treatment options, and lifestyle modifications to foster self-management and empowerment.
• Participate in community outreach and promotional activities to enhance patient engagement and awareness.
• Conduct home visits when necessary to provide in-depth care to patients unable to visit the clinic.
• Report directly to the Medical Director while collaborating closely with operational leadership to optimize service delivery.

Patient Population / Client Focus

The Adult Nurse Practitioner will primarily care for older adults on Medicare, focusing on maintaining their health and quality of life. The organization’s primary objective is to keep patients healthy and engaged in their communities through proactive care and personalized treatment plans.

Qualifications

• Possession of a Master's Degree in Nursing or a Doctor of Nursing Practice (DNP) or completion of a Nurse Practitioner Certificate Program.
• National certification as a Family, Adult, or Adult-Gerontology Primary Care Nurse Practitioner through organizations like AANP or ANCC.
• Active, non-probationary state Nurse Practitioner license and DEA license.
• Prior experience in primary care, including internal medicine, geriatrics, or family medicine.
• Familiarity with Hierarchical Condition Categories (HCC) documentation, ICD-10 Coding, and Health Risk Assessments (HRAs).
• A strong team orientation with a commitment to cross-functional collaboration.

Preferred Experience

• Previous involvement in innovative care models that prioritize population health management.
• Bilingual proficiency in another language relevant to our patient population.

Compensation and Benefits
