Gotham Enterprises Ltd is seeking a dedicated and experienced Senior Adult Nurse Practitioner (ANP) to join our team at our outpatient clinic in Elmont, New York. In this full-time role, you will play a critical part in delivering high-quality primary care services to our adult patients, particularly those on Medicare. Work Environment

This role is based in our outpatient office, where you will work alongside a diverse and multidisciplinary team that includes physicians, nurse practitioners, physician assistants, medical assistants, and dedicated medical scribes. This collaborative environment fosters a strong focus on primary care delivery, promoting care coordination and enhancing overall patient outcomes. Our clinic hours are Monday through Friday, 8:00 AM to 5:00 PM, allowing for a balanced professional lifestyle.

Key Responsibilities

• Provide outstanding primary care to a varied patient base, conducting comprehensive assessments and developing treatment plans.
• Perform routine office visits for both acute and chronic health issues, ensuring accurate documentation within our electronic health records system.
• Conduct annual wellness visits and health risk assessments, utilizing a holistic approach to healthcare.
• Facilitate care coordination with various healthcare providers, specialists, testing facilities, and agencies to streamline patient care pathways.
• Lead population health initiatives, engaging actively with in-house care teams to ensure essential screenings, such as breast cancer checks, are completed on schedule.
• Assist the care team with phone triage and manage patient outreach to improve access to health services.
• Prioritize patient education on health conditions, treatment plans, and lifestyle modifications, fostering empowering dialogue about their care.
• Engage in marketing and community outreach efforts to promote clinic services and enhance community connection.
• Be available for conducting home visits when necessary to accommodate patient needs.
• Report directly to the Medical Director, collaborating closely with operational leadership to improve clinic processes and patient care delivery.

Patient Population / Client Focus

Our primary focus is on older adults, particularly Medicare beneficiaries, as we emphasize personalized and compassionate care. The clinic’s innovative care model is designed to ensure these patients live healthy and fulfilling lives while being actively engaged in their health management. We aim to integrate care directly into the communities we serve, prioritizing quality over service volume.

Qualifications

• Master of Science in Nursing (MSN), Doctor of Nursing Practice (DNP), or completion of an accredited Nurse Practitioner Certificate Program.
• National certification as a Family, Adult, or Adult-Gerontology Primary Care Nurse Practitioner through AANP or ANCC.
• Possess a valid, non-probationary Nurse Practitioner license in New York State and an active DEA license.
• Hands-on experience in primary care settings; familiarity with internal medicine, geriatrics, or family medicine is strongly preferred.
• Knowledgeable in Hierarchical Condition Categories (HCC) documentation, ICD-10 coding, and health risk assessments, to ensure accurate reporting and care planning.

Preferred Experience

• Proven ability to work effectively in a collaborative team environment.
• Understanding and commitment to evidence-based population health strategies.
• Bilingual skills are highly valued; please specify applicable languages.

Compensation and Benefits

The annual salary for this position ranges between USD 180,000 and 220,000, depending on experience and expertise. Additionally, we offer a comprehensive benefits package, which includes:
