Position Overview

Gotham Enterprises Ltd. is seeking a dedicated and skilled Advanced Cardiac Care Nurse Practitioner (NP) to join our dynamic outpatient practice at the Center for Advanced Cardiac Care (CACC) located in Springfield Gardens, New York. This full-time, onsite position offers a competitive salary range of USD 120,000 – 220,000 annually, commensurate with experience. The NP will have the invaluable opportunity to work under the management of the Chief Nurse Practitioner, alongside the CACC Medical Director within the Division of Cardiology.

The outpatient office is equipped with state-of-the-art technology, including advanced electronic medical records (EMR) systems, allowing for efficient patient documentation and communication.

Key Responsibilities

As a Nurse Practitioner in the CACC, your day-to-day responsibilities will include:

• Conducting thorough patient history interviews and performing comprehensive and targeted physical examinations.
• Ordering and interpreting diagnostic tests to support the management and treatment of both chronic and acute patient conditions.
• Collaborating with physicians and NP colleagues to manage specific patient populations, fostering a culture of patient-centered care.
• Coordinating hospital admissions and ensuring effective communication with inpatient teams to facilitate safe patient discharges.
• Documenting patient interactions and treatment plans in the EMR, ensuring accuracy and confidentiality.
• Delivering patient education on disease processes, care management strategies, and individualized treatment plans.
• Participating in clinical research protocols, reviewing the latest findings, and integrating relevant research into everyday practice.

Patient Population / Client Focus

The Nurse Practitioner will focus primarily on a diverse population of patients with a variety of cardiac conditions, including those with advanced heart failure, pulmonary arterial hypertension, and individuals undergoing evaluation for heart transplantation. Additional responsibilities include supporting patients receiving outpatient intravenous therapy or advanced therapies and providing comprehensive education to patients and their caregivers regarding complex disease management.

Qualifications

To thrive in this role, candidates must possess:

• A Master’s degree in Nursing with Nurse Practitioner training specifically focused on adult care.
• A minimum of two years of clinical experience in a hospital or outpatient setting, preferably in Cardiology.
• Current New York State Registered Professional Nurse and Nurse Practitioner licenses.
• NP Board certification, along with certifications in BCLS, ACLS, DEA, and NPI.

Preferred Experience

While the aforementioned qualifications are essential, the following experiences are preferred:

• Hands-on experience working with advanced heart failure patients.
• Familiarity with the management of patients receiving outpatient intravenous medication therapy.
• Experience collaborating with multidisciplinary teams, enhancing care coordination and continuity.
• Proficient use of advanced electronic medical record systems and Microsoft Office Suite applications.
