Jewish Community Health Center is actively seeking a dedicated and skilled Nurse Practitioner to join our esteemed Center for Advanced Cardiac Care (CACC) team in Brooklyn, New York. This full-time position offers a unique opportunity to deliver high-quality, patient-centered care in a concierge medicine environment, focusing on outpatient cardiac health. The NP will work under the leadership of the Chief Nurse Practitioner and collaborate closely with our multidisciplinary cardiology team, contributing to the overall well-being and health of our patients.

Work Environment

This position is onsite, requiring work within our state-of-the-art facility dedicated to concierge medicine and house calls. The CACC department fosters a collaborative atmosphere where Nurse Practitioners, Physicians, and nursing staff support each other in delivering coordinated care, ensuring that all patients receive individualized attention tailored to their specific health needs. The normal work schedule is Monday to Friday from 9 am to 5 pm, with no requirements for calls, nights, or weekends, allowing for a healthy work-life balance.

Key Responsibilities

• Collaborate with board-certified cardiologists to manage a diverse patient caseload, ensuring comprehensive evaluations and treatment plans for patients.
• Conduct thorough patient assessments, including history-taking and physical examinations, to identify health issues accurately.
• Manage chronic conditions such as hypertension and hyperlipidemia, adapting treatments based on individual patient needs and current clinical guidelines.
• Provide specialized outpatient Electrophysiology services, addressing abnormal electrocardiograms and arrhythmias with competence.
• Oversee the care of patients diagnosed with acute cardiovascular conditions like Coronary Artery Disease, Congestive Heart Failure, and more, implementing effective management strategies.
• Perform stress testing when clinically indicated and interpret results to guide treatment decisions.
• Ensure continual professional development through active participation in rounds and consultations with on-site cardiologists.
• Prescribe medications and create treatment plans as directed, ensuring adherence to best practices and safety protocols.
• Educate patients and their families about cardiac conditions, treatment options, and lifestyle modifications essential for better health outcomes.
• Refer patients to specialists as necessary, promoting coordinated care within our healthcare network.

Patient Population / Client Focus

The Nurse Practitioner in this role will serve a diverse patient population, including individuals with both chronic and acute cardiac conditions. You will be responsible for developing and maintaining long-term relationships with patients, providing continuity of care as you address complex health issues and facilitate chronic disease management. Our focus on outpatient care allows for meaningful interactions where time is dedicated to understanding patients’ needs, promoting better self-management, and empowering them in their health journeys.

Qualifications

• A Bachelor’s degree in nursing is required; a Master’s or Doctoral degree is preferred.
• Successful completion of an accredited Nurse Practitioner program with extensive clinical training.
• Current, unencumbered Nurse Practitioner licensure in the state of New York is mandatory.
• Certification from a recognized board (e.g., ANCC or AANP) in relevant specialty areas.

Preferred Experience

We invite applications from both seasoned Nurse Practitioners and recent graduates. Ideally, candidates will have experience with outpatient cardiology, but we also value passionate individuals with a willingness to learn and grow within the specialty. Previous involvement with conditions related to electrophysiology and arrhythmias will be seen as an asset.

Compensation and Benefits

The salary range for this position is competitive, between USD 120,000 and 160,000 per year, commensurate with experience and qualifications. In addition to a substantial salary, our comprehensive benefits package includes:
