Gundersen Health and Wellness Clinic is actively seeking a skilled and compassionate Nurse Practitioner (NP) to join our team at the Center for Advanced Cardiac Care (CACC) located in Hempstead, New York. In this full-time, onsite role, the NP will be instrumental in providing high-quality care to patients dealing with complex cardiac conditions. As part of a dynamic interprofessional team, the NP will collaborate with physicians, nursing staff, and various specialists to ensure comprehensive patient management from diagnosis to treatment.

Work Environment

This full-time position is based at an Assisted Living Facility, where you will work closely with the CACC Medical Director and the Chief Nurse Practitioner. Key Responsibilities

As a Nurse Practitioner in our Cardiology Clinic, you will:

• Conduct thorough patient assessments, including obtaining detailed medical histories and performing comprehensive physical examinations.
• Order and interpret diagnostic tests such as EKGs, echocardiograms, and lab work to diagnose and manage both acute and chronic cardiac conditions.
• Develop goal-oriented care plans that adhere to established protocols guiding nursing and medical interventions.
• Administer and manage treatments under supervision, with the capacity to facilitate care autonomously as well.
• Document all patient encounters meticulously in our electronic medical records (EMR) system, ensuring accurate tracking of treatment plans and patient progress.
• Collaborate with the multidisciplinary care team—encompassing social workers, nutritionists, pharmacists, and surgeons—to provide holistic care across the continuum of health services.
• Engage in ongoing education with patients and caregivers, informing them about disease processes and strategies for effective care management outside the clinical setting.
• Participate in initiatives that monitor and enhance patient safety and quality of care within the facility.
• Participate in clinical research protocols, including the dissemination of findings that can benefit overall patient care in cardiology.

Patient Population / Client Focus

The Nurse Practitioner will primarily serve a diverse patient population, focusing on individuals with cardiac conditions such as heart failure, pulmonary arterial hypertension, and those who are post-heart transplant. This role also encompasses patients undergoing advanced therapies, realizing the full importance of high-quality care tailored to complex cardiovascular demands.

Qualifications

The ideal candidate for this position will possess:

• A Master’s degree in Nursing with specialization as a Nurse Practitioner, particularly in adult care.
• A valid New York State Registered Professional Nurse and Nurse Practitioner license.
• Board certification for Nurse Practitioners, ensuring you meet the professional standards in the field.
• Certifications in Basic Life Support (BCLS), Advanced Cardiac Life Support (ACLS), Drug Enforcement Administration (DEA), and National Provider Identifier (NPI).
• Proficiency with essential computer skills including experience with electronic medical record systems and familiarity with Microsoft Office Suite (Word, PowerPoint, and Excel).

Preferred Experience

While a minimum of two years of clinical experience is required, candidates with a background specializing in Cardiology and familiarity with advanced heart failure management, including outpatient intravenous therapies, are highly preferred. Experience in coordinating care for patients requiring mechanical circulatory support will be advantageous.

Compensation and Benefits

The Nurse Practitioner position offers a competitive annual salary ranging from USD 120,000 to 220,000, tailored to reflect your unique skills, experience, and qualifications.
