Work Environment

Key Responsibilities

• Conduct thorough assessments of patients' health status, including their medical history, physical examinations, and necessary diagnostic tests.
• Prescribe medications and treatment plans based on the unique needs of each patient, ensuring safety, efficacy, and cost-effectiveness.
• Implement health promotion and disease prevention strategies, educating patients on lifestyle modifications and behaviors to mitigate health risks.
• Provide guidance to patients on managing chronic illnesses, helping them develop personalized self-management plans.
• Utilize Electronic Health Records (EHR) effectively to document patient interactions, treatment plans, and outcomes, adhering to policies and regulations.
• Regularly evaluate and monitor patients' conditions to adjust treatment plans as necessary and ensure optimal health outcomes.

Patient Population / Client Focus

In this role, you will serve a diverse patient population, ranging from children to the elderly, each with unique health concerns. Your ability to empathize and connect with patients through personalized care will foster trust and improve health outcomes. You will address chronic health issues, such as diabetes and hypertension, as well as acute conditions, ensuring comprehensive care that resonates with their individual backgrounds and lifestyles.

Qualifications

• Current national certification from AANP or ANCC, demonstrating your competency and expertise in the field.
• An active and unrestricted Nurse Practitioner license in the state of New York is required.
• A Bachelor's degree in nursing is the minimum educational requirement, with a Master of Science in Nursing or a Doctor of Nursing Practice preferred.
• Eligibility for U.S. work authorization.

Preferred Experience

• Previous experience in a home care or outpatient clinical setting is highly desirable.
• Demonstrated ability to work in a flexible environment, efficiently managing your time to meet patient needs.
• Experience using electronic health record systems to enhance patient care documentation and tracking.

Compensation and Benefits

We offer a competitive salary range of USD 150,000 to 160,000 per year, commensurate with experience and caseload.
