Work Environment

This full-time position is onsite, allowing for direct interactions with patients in a home care setting. Key Responsibilities

• Conduct comprehensive patient assessments, which include reviewing medical histories, performing detailed physical examinations, and ordering necessary diagnostic tests to evaluate health status.
• Diagnose and manage acute and chronic health conditions, providing timely interventions to promote optimal patient outcomes.
• Order, interpret, and follow up on diagnostic tests such as blood work, imaging, and other necessary screenings to ensure accurate diagnosis.
• Develop personalized treatment plans based on thorough patient assessments and diagnoses, effectively adapting plans as patient needs evolve.
• Prescribe medications, therapies, and medical interventions, ensuring patients receive appropriate pharmacologic treatments for their conditions.
• Monitor patient progress and health conditions, making necessary adjustments to treatment plans to achieve optimal health outcomes.
• Provide education to patients on health maintenance, disease prevention strategies, and lifestyle modifications, empowering them to take control of their health.
• Work collaboratively with multidisciplinary teams of healthcare professionals, including physicians, nurses, and specialists, to offer holistic care tailored to individual patient needs.
• Refer patients to appropriate specialists for advanced care as necessary, facilitating a seamless continuum of care.
• Perform essential medical procedures, including but not limited to suturing, wound care, vaccinations, and biopsies, as part of patient treatment.
• Stay current with the latest medical advancements, research, and practices through ongoing education, contributing to the quality of care provided.
• Participate in research initiatives and quality improvement projects, driving evidence-based practices and innovation within the clinic.
• Mentor nursing students and colleagues, and actively participate in educational programs aimed at fostering the professional growth of the nursing team.

Patient Population / Client Focus

Our Nurse Practitioner will primarily serve adult and geriatric patients, with a focus on addressing various medical conditions ranging from preventive health care to chronic disease management. Understanding the unique challenges of this patient demographic is essential, as we aim to support their individual health journeys through continuous education and empowerment.

Qualifications

• Completion of a Bachelor of Science in Nursing (BSN) from an accredited institution.
• Completion of a Master of Science in Nursing (MSN) or Doctor of Nursing Practice (DNP) degree with specialization in a Nurse Practitioner program.
• Possession of a valid state Advanced Practice Registered Nurse (APRN) license and NP certification in a relevant specialty (i.e., Family, Adult-Gerontology, Pediatric).
• Demonstrated proficiency in performing patient assessments, developing diagnoses, and implementing treatment plans.
• Exceptional communication and interpersonal capabilities, emphasizing the importance of a patient-centered approach to care.

Compensation and Benefits

This position offers a competitive annual salary ranging from $93,600 to $140,000, commensurate with experience and qualifications.
