Work Environment

This position operates primarily onsite, allowing for direct interaction with patients in a comfortable and familiar setting. The schedule is Monday through Friday from 9:00 AM to 5:00 PM, offering a work-life balance while enabling you to make a significant impact in your patients’ lives. As part of a concierge medicine model, you will engage with patients during visits that prioritize time, attention, and personalized care, deviating from the rushed atmosphere often found in traditional healthcare environments.

Key Responsibilities

• Conduct thorough patient assessments, including comprehensive medical history reviews and detailed physical examinations within the home setting.
• Diagnose and manage both acute and chronic health conditions, utilizing evidence-based practices to inform clinical decisions.
• Create and implement individualized treatment plans, prescribing medications and coordinating follow-up care as needed.
• Engage patients in preventive healthcare practices and provide education on lifestyle modifications to promote overall wellness.
• Collaborate with physicians and other healthcare professionals to ensure continuity and quality of care for each patient.
• Document all patient interactions and care interventions accurately in electronic health records (EHR) to maintain high standards of documentation and reporting.
• Stay updated with the latest healthcare trends, treatment protocols, and best practices to continuously improve patient care.

Patient Population / Client Focus

As a Nurse Practitioner at Northern Care Hospital, you will serve a diverse patient population that includes individuals with chronic conditions, preventive health needs, and acute episodic care requirements. Your role will focus on delivering tailored healthcare solutions, enhancing patient experience by providing care directly in their homes, thus fostering trust and open communication while addressing their unique healthcare needs.

Qualifications

• Master of Science in Nursing (MSN) or Doctor of Nursing Practice (DNP) from an accredited institution.
• Active and unrestricted Nurse Practitioner license in the state of Maryland, or eligibility for licensure.
• Board certification as a Family Nurse Practitioner (FNP) or Adult-Gerontology Nurse Practitioner (AGNP) is required.
• Current DEA registration and prescriptive authority.
• Proficiency in electronic health records (EHR) management systems to enhance efficiency in patient documentation.
• Exceptional clinical, diagnostic, and communication skills that build rapport with patients and their families.

Preferred Experience

• Minimum of 2 years of clinical experience as a Nurse Practitioner, preferably in primary care, internal medicine, or a specialty practice.
• Experience managing chronic diseases such as diabetes, hypertension, and cardiovascular issues.
• Demonstrated ability to work effectively within multidisciplinary teams to coordinate comprehensive patient care.
• Strong patient education and counseling skills to guide individuals in their health journeys.
