Work Environment

This role is set in an onsite hospital setting located in Baltimore, offering a vibrant atmosphere where collaboration and communication are paramount. The work hours are Monday through Friday from 9 AM to 5 PM, allowing for a stable work-life balance while providing ample opportunity to engage with patients and peers.

Key Responsibilities

• Conduct comprehensive assessments, formulate diagnosis, and develop treatment plans in collaboration with supervising physicians, ensuring high-quality care for patients.
• Provide effective patient consultations, explaining diagnostic information and treatment options clearly to empower patients to make informed decisions.
• Utilize evidence-based practice to perform routine and complex medical procedures while monitoring patient progress and adjusting treatment plans as necessary.
• Engage in preventative care practices, addressing disease prevention and health promotion initiatives pertinent to our primary patient population, including managing chronic conditions.
• Document patient encounters accurately in our electronic health record (EHR) system to ensure consistent communication across the healthcare team.
• Educate patients and their families on disease management strategies, preventative measures, and the importance of adherence to prescribed treatment, fostering a culture of proactive health management.

Qualifications

• A Bachelor’s degree in Nursing from an accredited institution is required.
• Possession of a valid and unrestricted Certified Nurse Practitioner (CNP) license in the State of Maryland, along with national certification as a Family Nurse Practitioner (FNP) from an accredited certifying organization.
• Minimum of two years of professional nursing experience, with a strong focus on internal medicine or primary care settings.
• While acute care certified NPs are fantastic professionals, this position specifically requires expertise in internal medicine, ensuring our focus on chronic and long-term care issues.

Preferred Experience

Experience in managing chronic disease processes, conducting health assessments, and developing patient-centered care plans is preferred. Compensation and Benefits
